L. That there be and hereby is created new Sections 1119.06 of the Zoning <br />Code of the City of North Olmsted, which sha11 read in its entirety as follows: <br />"1119.06 CERTIFICATE OF DIRECTOR OF FINANCE REQUIRED. <br />(a) The provisions of this Section of Chapter 1119 sha11 apply to applications for <br />occupancy permits involving commercial construction and new residential construction. <br />"Commercial construction" shall mean the construction of any building or other improvement on <br />land in the City which is zoned for any use other than single-family, two-family or single family <br />cluster. "New residential construction" shall mean the construction of any new residential building <br />on land which is zoned for single-family, two-family or single family cluster use. <br />(b) Every applicant for an occupancy permit whose application is subject to the <br />provisions of this Section sha11 obtain and report to the Director of Finance the following <br />information concerning every individual who was self-employed or employed by any developer, <br />owner, contractor or subcontractor, and who worked at the commercial construction or new <br />residential construction site located within the City for which the occupancy permit is sought: <br />(i) Name of Employee; <br />(ii) Address of Employee; <br />(iii) Social Security Number of Employee; <br />(iv) Name and Address of Employee's Employer; <br />(v) Dates the Employee worked in the City; <br />(vi) Gross Wages of the Employee for each date worked in the City. <br />The applicant shall further provide to the Director of Finance, on forms provided by the <br />Department of Finance: first, the applicant's written certification that a11 of the said information <br />provided concerning said employees is, to the best of the applicant's belief and knowledge, true and <br />complete, and secondly, written certification from the developer, owner, and each contractor and <br />subcontractor performing contractual services at the construction site, that the employer and <br />employee information, as it pertains to the employees of each such developer, owner, contractor, <br />and subcontractor, is true and complete to the best of his or her belief and knowledge. <br />(c) The Building Official shall not issue an occupancy permit as requiretl by Section <br />1119.02 of this Chapter to any applicant who is subject to the provisions of this Section 1119.06, <br />unless the Building Official has first received written certification from the Director of Finance that <br />the applicant has complied with all of the requirements of this Section set forth in paragraph (b) <br />above. <br />.SEC710AL 2: That any ordinance, or part of ordinance, which is in conflict with or is <br />inconsistent with this Ordinance, to the extent of any such conflict or inconsistency, is hereby <br />repealed. <br />
