ARTICLE XIV <br />SICK LEAVE <br />14.1 Sick leave shall be earned in accordance with the Ohio State minimum requirements and <br />accumulation shall be unlimited. <br />14.2 Line Personnel shall have the option to receive twenty six (26) hours compensatory time or twenty <br />six (26) hours pay at the Basic Hourly Rate of Pay for Line Personnel plus Longevity for each six <br />consecutive months of unused Sick Leave. Staff Personnel shall have the option to receive twenty <br />(20) hours compensatory time or twenty (20) hours pay at the Basic Hourly Rate of Pay for Staff <br />Personnel plus Longevity for each six consecutive months of unused sick leave. Selection of such <br />time off shall be subject to the approval of the Fire Chief or his designee. <br />14.3 An Employee or his estate shall be paid for unused, accumulated Sick Leave at the time of <br />disability, retirement or death. Payment shall be based upon the Basic Hourly Rate of Pay for his <br />"work week" at the time of disability, retirement or death plus Longevity and the amount shall be <br />one-half of the unused Sick Leave with a maximum payment of 1000 hours. <br />14.4 Any Employee who transfers from one depaztment within the City of North Olmsted to another <br />shalt be credited with the unased balance of Sick Leave accumulated. <br />14.5 Sick Leave may be taken for any of the following reasons: <br />A. Illness or injury to the employee. <br />B. Illness or injury of a member of the Employee's unmediate family. <br />C. Medical, dental, or oprical examination or treatment of the Employee, where the treaunent <br />may not be scheduled during non-working hours. <br />D. Exposure to contagious disease where quazantined by the Board of Health or communicable <br />to other Employees. <br />E. Pregnancy of the Employee where complications exist or childbirth by Employee's spouse. <br />14.6 The Employer shall require an Employee to furnish a standard form identifying the reason for the <br />use of sick leave as per 14.5 and signed by the Emptoyee. <br />14.7 If inedical attention is required, the Employee shall be reyuired to furnish a statement from a <br />licensed physician or psychologist notifying the Employer that the Employee was unable to <br />26 <br />,. <br />.,. .-
