PROPOSAL DEADLINE <br />Four (4) bound copies of a proposal must be received by the City of North <br />Olmsted, 5200 Dover Center Road, North Olmsted, Ohio 44070, no later than <br />4:00 p.m., January 10, 2003. All proposals must be addressed to the attention of <br />Ms. Mary E. Creadon, Director of Public Service. All proposals shall be plainly <br />marked on the outside "PROFESSIONAL DESIGN SERVICES PROPOSAL". <br />V. PROFESSIONAL DESIGN SCOPE OF SERVICES <br />The successful Proposer will provide professional design scope of services as <br />outlined in APPENDIX B to this RFP. The services will be provided under the <br />Agreement between Owner and successful Proposer. <br />VI. CONTENTS OF PROPOSALS <br />Statements of qualifications must be included with the Proposal and should inclu.de <br />information regarding: <br />A. Firms history; <br />B. Experience in providing professional design services for projects similar to <br />and larger than the Bradley Road and Windsor Drive Culvert Rehabilitation <br />Projects; <br />C. Education and experience of staff who will be involved in the Project; <br />D. (Study, evaluation, preliminary design, detailed design, pre-construction, <br />construction and/or post-construction) phase services offered by your firm.; <br />E. Firm's general liability insurance and financial responsibility; <br />F. List any pending or current legal proceedings such as mediation, arbitration <br />or suits; <br />G. References: <br />H. Other services offered by the firm; and <br />1. Documentation indicating firm's compliance or non-compliance with each of <br />the standards listed in Appendix E. <br />The Proposal shall include all information required to be submitted by the Request <br />for Proposal and any of its Appendices. The Proposer shall include a proposed <br />schedule of all services required as part of the professional design services. It is the <br />intent of the City to complete all construction in 2003. <br />Firms who wish to submit a proposal should follow the form of this Request for <br />Proposal. Please refer to Section VII - Subsection F for the specific requirements <br />for which the Proposer should use to document its qualifications since the specific <br />
