If it believes that an emp3oXee is <br />developing an attendance problem, it wi12 natify the <br />employee in writing that medxcal evidence will be <br />required for absences of three (3) days or less. When an <br />enployee's recard becomes satisfaatory, the requirement <br />fox such medical evidence for short terrn absence will be <br />removed, and the employee will be notifzed in writxng. <br />d) Employees may use up to fifteen (15) work days per yeax <br />of e.arned and aacumu3.ated sick leave if neaessitated by <br />i1.lness in tkze emp1aXee' S immediate family. Hawever, if <br />an employee uses more than six (G) sick work days per <br />calendar yeax for this purpose, the City may require that <br />the employee grovide a certificate fram the "immedxate <br />family" member's doctor(s), explaining the need and <br />purpose of the employee's absence. For purposes of this <br />subsection, "immediate family" shalx be interpreted to <br />incxude: spoase, children, parents di the employee - <br />regardless nf the place of residence, and othex relatives <br />living within the emp2oyee's househald. <br />e) When an employee with five (5) or maxe years of servi_ce <br />retires, having received P.E.R.S. pension and having <br />accumuZated sick leave time, the retirinq employee may <br />cdnvert one-half (1j2) of the accumulated s:t.ck leave time <br />ta pay on the basis of his rate of pay at the time of <br />retireme»t, but the maximum amount paic2 shall not ekceed <br />four hundred eighty (480) hours paid. <br />f} An employee should make an effart to schedul.e mediGal and <br />dental appointments for h.zmself and his family membexs <br />dur inq non-work hours. <br />g} Arty abuse af siGk leave or the unjustified patterned use <br />of sick leave shall be just and suffiGisnt aause for <br />discipline, including da.scharge, as may be determined by <br />the Employer. <br />ARTICLE 20: <br />SICI{ LEAVE BONITS <br />SECTI4N 1: All employees shall xeceive twv (2) eiyht (s) riour days <br />aff, wzth pay, or the employee may elect to receive sixteen (16) hours <br />pay for each sXx (6) cansecutive months af unused sic3t leave.
