Section 18.14. Sick leave can be used for the following reasons: <br />? :. (a) Sickness of the employee; <br />(b) Injury to the employee; <br />(c) Exposure to contagious disease where quarantined by the Board of Health; <br />(d) Emergency/non-routine medical, dental surgery or optical surgery of the <br />employee; and <br />(e) Sickness or emergency medical treatment of the employee's "immediate <br />family" shall be interpreted as meaning parents, pazents of spouse, spouse, <br />and children of the employee, regardless of place of residence, as well as <br />other relatives living in the employee's home. The maximum sick leave <br />which may be granted under, this paragraph for family illness may be no <br />more than eighty (80) hours per year for employees who have been with <br />the City more than twelve (12) months. Departrnent heads may require a <br />satisfactory medical certificate before paying any employee under this <br />paragraph. Falsification of either a written, signed statement or a <br />medical certificate shall be grounds for discipiinary action including <br />dismissal. <br />(f) Sick leave may be used to attend the funerai of a half-brother, half-sister, <br />brother-in-law or sister-in-Iaw, for a maximum of three (3) days. <br />Section 18.15. Any abuse of sick leave or the patterned use af sick teave sha11 be just and <br />sufficient cause for disciptine, including discharge, as may be determined by the Employer. <br />(Examp[e: Mondays, Fridays, day after payday, etc.) <br />Section 18.16. Any employee scheduied to work on a holiday who reports sick shall be charged <br />t?;a <br />sick leave for that holiday, provided the proper NOMBL medical form has been completed and <br />shall forfeit his right to receive the holiday pay. If the employee has no sick leave or does not <br />have the proper NOMBL medical form completed, he shall receive no pay for that day. <br />ARTICLE XIX <br />LEAVES OF ABSENCE & FUNERAL LEAVES <br />Section 19.01. All employees shall be entitled to take a personal leave of absence with the <br />approval of the Service Director and under the terms and conditions as he sets forth, which <br />approval shall not be unreasonably withheld. Any leave of absence for persona[ reasons, shatl not, <br />however, exceed a ninety (90) day period. Said employee in order to maintain any and all benefits <br />which are paid by the City shall be required to pay the Finance Director, the cost of said benefits <br />in advance on a monthly basis. The maximum permissibie leave of absence for any reason shal( <br />be ninety (90) days. <br />(a) Special circumstances may occur for an emptoyee to make application to the <br />Service Director for a leave of absence; whereby this leave will not effect the <br />employee's seniority as stated in Section 14.01. This situation will not be <br />precedent setting for past or future leave of absence requests granted by the city. <br />Section 19.02. Any empioyee who is requred to be away from work due to sickness shall be <br />? entit(ed to use up sick leave which has been accumulated, according to the rates as specified in <br />Sec. 18.05, and Sec. 18.06. When an employee has exhausted all accumulated sick leave, then <br />that employee shall be entitled to a sickness leave of absence which shall not exceed ninety (90) <br />15
