1. Dental <br />2. Vision - Optional Plan #1 (Vision Plan which was in effect August l, 1994) <br />Vision - Optional Plan #2 (Vision plan similar to that offered through <br />Union Eye Care) <br />3. Dental and Vision (Option Plan #1 or #2) <br />Employees participating and subscribing to the optional health benefits listed <br />above within this section, shall contribute an amount equal to that which is listed within <br />Section 112 of this Article. Employees must participate in the medical plan to be <br />permitted to participate within the optional benefit coverage listed within this section. <br />The employee contribution shall be withheld as a payroll deduction not later than the <br />first pay period of each month. <br />11.4 Expected cost or premium is defined as the cost of all medical, hospital, <br />prescription drug, and related fees including, but not limited to administrative fees. <br />11.5 The employer reserves the right to establish all plans, enrollment periods and <br />regulations. <br />11.6 The Employer shall make an earnest effort to establish a committee consisting of <br />one (1) representative from each of the City bargaining units, if they choose to be <br />represented and a minimum of one (1) representative of the employer. The bargaining <br />unit's role as a participant in the aforementioned committee shall be advisoryonly. <br />11.7 All changes affecting the plan coverage's co-payments, deductibles, providers, <br />enrollment periods and regulations will be shared with the bargaining unit prior to <br />making such changes. <br />_.?.,. <br />15 <br />
