Any employee who transfers from one department within the City <br />to another shall be credited with the unused balance of sick leave <br />accumulated. This shall in no wav be construed as referring to any <br />other city than North Olmsted. <br />B. Use of Sick Leave <br />Employees may use sick leave for absence due to <br />1. Personal illness, injury, or exposure to contagious disease where <br />quarantined by the Board of Health. <br />2. Illness in immediate family with a maximum 15 work days per year. <br />The term "immediate family " in case of illness shall be interpreted <br />as meaning parents, regardless of place or residence, and relatives <br />living within the house hold of the employee. <br />3. Each death in the immediate family, a maximum of five work days, <br />including travel time. The term "immediate family " in the case <br />of death shall be interpreted as meaning parents, parents of spouse, <br />spouse, and children of the employee, regardless of place of <br />residence, as well as other relatives living in the employee's home. <br />4. Each death, of brother, or sister of employee, or spouse, five work <br />days. Necessary travel time would be included in that period. <br />C. Proof of Absence <br />The Department Head shall require that the employee furnish a satis- <br />factory affidavit to the effect that the absence was caused by illness, <br />or due to any of the foregoing causes. <br />D. Return to Full Time Duty <br />Employees who have been absent for three days or more, due to <br />illness, shall be required to present a statement from a physician <br />validating that they are physi cally able to resume their duties on <br />full-time basis. <br />When an employee has been absent for less than three days, the <br />Department Head, at his discretion, may request a statement from <br />the employee or his physician before the employee may return to <br />work. <br />At the discretion of the Department Head, Director or Mayor, the <br />City Physician may be requested to call upon any City employee. <br />In the event any employee has been off duty for a serious ailment, <br />such as a heart attack, or any employee has suffered loss of limb, <br />sight, physical or mental disability, and if any such employee is <br />unable to perform the duties,. he or she performed prior to his or her <br />disability, the Director or Mayor may deem it necessary to have <br />the employee approved by the City Physician before said employee <br />goes back to work. Decision as to whether the -employee is capable <br />
