General Leave Provisions. <br />,?. (a) Al1 leaves of absence when known in advance <br />(and any extensions thereof) must be applied <br />for and granted in writing on forms provided <br />by the City (copy to the employee). An <br />employee will be notified within three (3) <br />? working days from the date the application <br />was made. <br />? (b) An employee shall accumulate seniority during <br />any leave of absence, but he shall not be <br />entitled to any benefits, except hospital/ <br />surgical benefits which shall be provided by <br />the City on a fifty percent (50%) basis, <br />limited to a three (3) month period after an <br />initial thirty (30) day waiting period.
