37. Paid Sick Leave. An employee who has completed his <br />probationary period shall be granted sick leave for personal <br />illness or injury as follows: <br />(a) Paid sick leave shall be earned and <br />accumulated at the rate of one and one- <br />fourth (1-1/4) days for each month; <br />(b) Pay for sick leave shall be at the <br />employee's straight time hourly rate <br />(or nortion thereof if absent for less <br />than a full day); <br />(c) If an employee is absent on sick leave <br />for more than three (3) consecutive work <br />days, the City shall require the employee <br />to present medical evidence that the <br />employee was under a doctor's care and is <br />able to return to work. The City will <br />review the attendance records of employees <br />periodically and at least once in each <br />three (3) month period. If it believes that <br />an employee is developing an attendance <br />problem, it will notify the employee in <br />writing that medical evidence will be required <br />for absences of three (3) days or less. <br />When an employee's record becomes satisfactory, <br />the requirement for such medical evidence <br />for short term absences will be removed, <br />and the employee will be notified in writing. <br />(d) Employees may use, when necessary, up to <br />five (5) days of earned and accumulated <br />sick leave for the purpose of attending the <br />funeral of a member of their immediate family. <br />The term "immediate family" for purposes of <br />this subsection shall mean: parents; parents <br />of spouse; spouse; children of the employee; <br />brother; sister; brother of spouse, sister of <br />spouse; grandparent; grandchild; as well as <br />relatives living in the employee's home. <br />(e) Employees may use up to fifteen (15) work <br />days per year of earned and accumulated <br />sick leave if necessitated by illness in the <br />employee's immediate family.
