The GRANTEE shall monitor and report program performance for all grant <br />supported activities to assure that time schedules are being met, proposerl <br />program activities are beang accomplished, funds are being properly <br />expended and other performance objectives are being a.chieveci. The two <br />types of performaT.ice reports required to be submitted by the GRAN"EE uncler <br />this agreement are an activity performance report and a financial status <br />report. <br />a. Activity Performance Report <br />Ttle activity performance report sha.ll describe tlie extent of progress <br />for each activity identified in the grant program, any delays <br />encountered, any requests for changes in activity performance, or <br />anything else appropriate for determining adequate progress on the <br />grant program. The activity performance report should also identif__y <br />the activities or projects accomplished with the use of any cash <br />donations received for tax credit purposes. <br />b. Financial Status Report <br />A financial status report shall also be submitted and shall pravide a <br />clear and concise accounting of project receipts, costs and other <br />expendittres including expenditures of cash donations received for tax <br />credit purposes. It is required tha.t the GZANTEE use the financial <br />status report format available from the Office of Litter Control for <br />use in submi.tting this report. A final £inancial report covering the <br />entire grant period shall be submitted in addition to the last peri_od <br />report within fifteen (15) days o E the grant termination date. <br />I. <br />2. The activity performance reports and financial status reports shall be <br />submitted to: <br />GRAN'CS ADMIPIISTRA'POR <br />OFFICE OF LITTER CON'IROL <br />OHIO DEPAltUIENT OF NATLTRAL RESOURCES <br />F(XJN'I'AIN SQUARE COtJRT <br />COLUMBUS, Of IIO 43224. <br />Program Awareness and Aclmowledge.ment of Financial Assistance <br />l. All promotional materials, advertising and awards made available as a <br />result af this grant shall include the Ohio Litter Control Program logo and <br />tiiemeline. Arttvork for t'iie logo is available fram the Office of Litter <br />Control upon request. <br />2. All publications, brochures, etc. financed in whole or in part by this <br />grant shall aclmowledge that financial assistance was provided by the <br />Office of Litter Control, Ohi:o Department of Natural Resources. <br />J. Site Visits <br />The Office of Litter 0-ontrol reserves the right to conduct announced site <br />visits with the GRATTT'EE as arranged between the respective parties. 'rhese <br />visits will be conducted to review program progress, visually inspect activit_y <br />performance, discuss any stumbling blocks encountere(3 in project <br />implementation, receive any recomnendations or requests for project changes, or <br />to consider anything else mutually beneficial to program implementation. 'rhey <br />should be viewed positively as opportunities for direct, on-site corminication <br />between the Office of Litter Control and the GRANTEE.
