<br />this subsection, "immediate family' <br />shall be interpreted to be spouse; children, <br />parents of the employee, regardless of <br />the place of residence; and other <br />relatives living within the household <br />of the employee. <br />(f) When an employee with five (S) or more <br />years of service retires, having received <br />P.E.R.S. pension and having accumulated <br />sick leave time, the retiring employee may <br />convert one-quarter (114) of the accumu- <br />lated sick leave time to pay on the basis <br />of his rate of pay at the time of retire- <br />ment, but the maximum amount paid shall <br />not exceed two hundred and forty (240) <br />hours p ay. <br />(g) An employee should make an effort to schedule <br />medical and dental appointments for himself <br />and his family members during non-work hours. <br />38. General Leave Provisions. <br />(a) All leaves of absence when known in advance <br />(and any extensions thereof) must be applied <br />for and granted in writing on forms provided <br />by the City (cony to the employee). An employee <br />and the Union will be notified within three (3) <br />working days from the date the application <br />was made of approval or denial of the request <br />leave. <br />(b) An employee shall accumulate seniority during <br />any leave of absence, but he shall not be <br />entitled to any benefits, except hospital/ <br />surgical benefits which shall be provided by <br />the City on a one hundred percent (100%) basis, <br />minus ten dollars ($10) per month deducted <br />from first pay of each month, limited to a <br />three (3) month period after an initial thirty <br />(30) day waiting period. An employee must direct <br />pay any required hospital/surgical premiums to the <br />City on or before the monthly due date. <br />(c) When an employee returns to work after a <br />leave of absence, he will be assigned to the <br />nosition which he formerly occupied at the <br />then current rate of pay or to a similar position <br />if his former position no longer <br />-21- <br />? <br />_ . s,?
