cost which exceeds an average of $580.00, shall be withheld as any additional payroll deduction within (30) thirty days following determination of the actual average cost. 11.6 The employer will make an earnest effort to establish a committee. consisting of (1) one representative from each of the City bargaining units, if they choose to be represented and a minimum of (1) one representative of the employer. This bargaining unit's role as a participant in the aforementioned committee shall be advisory only. The employer reserves the right to establish all enrollment periods and regulations. All changes affecting coverage or chosen providers shall be shared with the bargaining unit prior to the making of such changes. 11.7 The employer shall pay the full amount of dental insurance for all full-time employees and dependents if employee chooses a family plan. The features of the plan will be consistent and similar to those in effect on December 1, 1991. Some of its features being a $25.00 per person per calendar year deductible together with an annual maximum of one thousand dollars ($1,000) payable per covered person during any benefit year. 11.8 The employer shall pay the full amount of vision insurance for all full-time employees and dependents if employee chooses a family plan. The features of the plan will be similar and consistent with those in effect December 1, 1991. 11.9 The employer will secure a commercial insurance policy whereby any bargaining unit member killed in the line of duty shall receive to his spouse or estate a payment of $50,000. Such payment will be made in accordance with the terms of the commercial plan. 11.10 Expected Cost or Premium is defined as the cost of all medical, hospital, prescription drug costs, and related fees. The employer reserves the right to establish all enrollment periods and regulations. (17)
