cost which exceeds an average of $580.00, shall be withheld as <br />any additional payroll deduction within (30) thirty days <br />following determination of the actual average cost. <br />11.6 <br />The employer will make an earnest effort to establish a <br />committee. consisting of (1) one representative from each of the <br />City bargaining units, if they choose to be represented and a <br />minimum of (1) one representative of the employer. This <br />bargaining unit's role as a participant in the aforementioned <br />committee shall be advisory only. The employer reserves the <br />right to establish all enrollment periods and regulations. <br />All changes affecting coverage or chosen providers shall <br />be shared with the bargaining unit prior to the making of such <br />changes. <br />11.7 <br />The employer shall pay the full amount of dental <br />insurance for all full-time employees and dependents if <br />employee chooses a family plan. The features of the plan will <br />be consistent and similar to those in effect on December 1, <br />1991. Some of its features being a $25.00 per person per <br />calendar year deductible together with an annual maximum of one <br />thousand dollars ($1,000) payable per covered person during any <br />benefit year. <br />11.8 <br />The employer shall pay the full amount of vision <br />insurance for all full-time employees and dependents if <br />employee chooses a family plan. The features of the plan will <br />be similar and consistent with those in effect December 1, <br />1991. <br />11.9 <br />The employer will secure a commercial insurance policy <br />whereby any bargaining unit member killed in the line of duty <br />shall receive to his spouse or estate a payment of $50,000. <br />Such payment will be made in accordance with the terms of the <br />commercial plan. <br />11.10 <br />Expected Cost or Premium is defined as the cost of all <br />medical, hospital, prescription drug costs, and related fees. <br />The employer reserves the right to establish all <br />enrollment periods and regulations. <br />(17) <br />
