(e) Evidence of Compliance. All applications for local license and registration certificate <br />shall include the following documentation, to be supplied by the applicant: <br />(1) A certificate of insurance for general liability for bodily injury and property <br />damage in the amount of $250,000.00; <br />(2) A certificate of insurance for construction claims and unworkmanlike <br />performance in the amount of $250,000.00; <br />(3) A certificate of coverage from the Bureau of Workers' Compensation; and <br />(4) Federal tax or payroll identification number or Regional Income Tax Account <br />number or other evidence of registration and/or certification of compliance with <br />RITA withholding. <br />(5) A certificate of compliance with Renovation, Repair and Painting (RRP) Rules <br />issued by the Environmental Protection Agency (EPA) for lead-based paint and/or <br />window replacement, where applicable. <br />(f) Issuance of Certificate. The Building Commissioner, upon receipt of all required fees, <br />documentation and evidence of compliance necessary to satisfy the provisions herein, shall issue <br />or deny a certificate of registration for a state licensed contractor pursuant to subsections (c) and <br />(e); shall issue or deny a local license certificate pursuant to subsection (d) and (e); or shall issue <br />or deny both forms of certification to qualified applicants. An application shall not be deemed to <br />have been approved until the Building Commissioner has examined such application, determined <br />therefrom that the applicant has satisfied the requirements of Section 1317.04, and has issued a <br />certificate for local license or a certificate for registration of state licensee or both. <br />(g) Appeals. The applicant for a local license or registration certificate may appeal from <br />the denial by the Building Commissioner to the Board of Building Code Appeals. The Board <br />shall hear and decide the appeal at its next regular meeting. At the discretion of the Board of <br />Building Code Appeals, after a review of records and an oral examination, a temporary local <br />license certificate may be issued but not to exceed two years in duration. After the two-year <br />probationary period, the applicant may reapply to the Building Commissioner and, upon appeal, <br />the Board shall either deny or affirm the application. An applicant for a local license certificate <br />or a probationary local licensee may obtain a state license or certification and also apply for <br />registration thereof under subparagraph (c), which registration shall be independent of the local <br />license requirement with respect to the work or trade so licensed by the State. <br />(h) Records. The Building Commissioner shall keep a record of all certificates of <br />registration-and certificates for local licenses,-and together with other documentation forming the <br />basis for the issuance, or the denial thereof, on file for not less than three years. <br />1317.05 GENERAL QUALIFICATIONS. <br />(a) Notwithstanding an applicant's compliance with Section 1317.04, no contractors shall <br />be issued a local license certificate, nor shall such license be renewed, if the applicant lacks good <br />moral character. In any determination as to the moral character of an applicant, the Building <br />Commissioner shall consider the applicant's record of violations of the Building Code, of other <br />laws or ordinances regulating building construction in the City, and evidence of similar <br />violations in other communities. Evidence of dishonest construction practices or malpractice in <br />the conduct of a business, trade or profession, shall also be considered as evidence of moral <br />character or the lack thereof. Upon receipt and review of such evidence, the Building <br />Commissioner may determine that it is in the public interest that such applicant not be issued a <br />certificate of local license or that same not be renewed and thereby refuse or revoke authority to
