license, as required by subparagraph (a), shall be guilty of a misdemeanor of the first degree and <br />subject to prosecution therefor, in addition to any other sanctions imposed herein. <br />1317.03 PUBLIC UTILITY ORGANIZATIONS' EXCEPTION. <br />No provision of this chapter of the Building Code shall be interpreted to require licenses, <br />registration or certificates of qualification for officials or employees of public utility <br />organizations to engage in the installation, alteration, repair, maintenance or utilization of any <br />device, appliance, installation or appurtenance forming part of the equipment for generation, <br />transmission or distribution of any commodity or service which such public utility organization <br />is authorized by law to furnish or provide. Public utility organizations, as exempted herein, shall <br />be limited to those utilities whose activities are regulated by the Public Utilities Commission of <br />Ohio (PUCO) or are otherwise exempt from municipal regulation by operation of federal law. <br />1317.04 APPLICATIONS. <br />(a) Forms. The Building Commissioner shall establish and direct the Building <br />Department on the use of uniform application forms for local license and registration certificates. <br />(b) Sworn Application. All applications for local licenses and registration certificates for <br />which a fee is prescribed, shall be sworn to by the applicant before a notary public. Upon <br />reasonable standards determined by the Building Commissioner, renewal forms may be exempt <br />from notarization. <br />(c) Registration Submittals. All applicants for a certificate of registration must provide <br />evidence of compliance with Ohio Revised Code Chapter 4740 et seq., which shall be deemed <br />satisfactory upon the presentation of documentation demonstrating a current license or certificate <br />issued by the State of Ohio in a form under seal or a copy thereo£ No further or additional <br />testing, review or evidence of skill shall be required of a contractor demonstrating such <br />compliance with said state law, provided however, that the evidence of compliance is also <br />submitted as required in subparagraph (e) herein. <br />(d) License Submittals. All applicants for a local license must provide satisfactory <br />evidence of skill in the work or trade for which a license is sought, together with other <br />documentation required herein. Satisfactory evidence of skill must be demonstrated to the <br />Building Commissioner, by submission of any or all of the following: <br />(1) An affidavit from the applicant which describes the manner in which he has <br />been actively engaged in the licensed area for a period of five or more years and <br />which otherwise describes his expertise, including formal education, testing or <br />specialized training and certification; <br />(2) A complete list of the names, addresses and phone numbers of clients or other <br />contractors for whom they have performed work in this or any trade and a <br />description of the projects involved; <br />(3) If applicable, a current license or certification by the State of Ohio in any field <br />or trade; <br />(4) If applicable, a current license or certificate issued by any trade council, <br />testing institution, accredited board or other trade group, together with <br />examination results; and <br />(5) A copy of any licenses or registration certificates issued by any other state or <br />municipality, including any examination results.
