Ridge , MeKenzie Read, Mastiek Read, Mill Read, PeAer- Read, Steafns Read, <br />, ," <br />: $1,500 <br />901.02 ISSUANCE REQUIREMENTS. <br />No such permit shall be issued until: <br />(a) The applicant shall have made the deposit referred to in Section 901.01 or 901.07. Such <br />permit shall state the exact time the opening or excavation is to be made, the probable length of <br />time for doing the work and the making of the repair, the character of the pavement, including its <br />base, and the character of the work necessary for the satisfactory repairing or relaying of the <br />roadway. <br />(b) The applicant shall have submitted a policy or policies of liability or indemnity insurance <br />issued by companies licensed to do business in the State, recognizing the City as an insured, in a <br />total amount of not less than five hundred thousand dollars/one million dollars <br />($500,0001$1,000,000) for any personal injury or injuries incuned and two hundred thousand <br />dollars ($200,000) for any property damage caused as a result or consequence of the making or <br />existence of such opening or excavation, or the condition of the road, street, sidewalk, tree lawn <br />or other public property in which the same is or has been made. <br />901.03 NOTIFICATION OF POLICE DIVISION. <br />Upon the issuance of a permit as set forth in Section 901.02, it shall be the duty of the City <br />Engineer to advise the Police Division of the location, time and duration of the street opening, in <br />order that preparation can be made by the Police Division for traffic control in the area of the <br />street opening. <br />901.04 CONTRACTOR TO MAKE STREET REPAIRS; FAILURE TO COMPLY. <br />(a) Upon completion of the work authorized by the permit granted by compliance with the <br />provisions of this chapter, the contractor shall make such temporary repairs as are necessary to <br />maintain traffic and shall promptly repair the roadway and/or right of way as soon as possible. In <br />the event the contractor fails to make such repairs within ten days after notice from the City, the <br />City may cause such repairs to be made and the cost deducted from the deposits or invoiced to <br />the public utility company as required by Section 901.01. In the event of an emergency or <br />condition causing a serious hazard to the public, the Service Director may cause such hazard to <br />be immediately abated and the cost of such abatement shall be deducted from the deposits. <br />(b) All repairs shall be done to the satisfaction of the City Engineer. The City Engineer shall <br />apply performance standards consistent with engineering and other construction industry <br />standards and may adopt additional specific policies, if necessary, related to the backfill and hard <br />surface requirements for restoration of excavated areas.
