Cuyahoga County Board of Health Programs and Services <br />''e Cuyahoga County Board of Health (CCBH) offers a wide range of public health services to the citizens and <br />communities in our health district. Some of these services may require income qualification or other qualifying <br />criteria. The focus of some of our grant related activities may be specific, based on guidance from our funding <br />agencies. The following is a list of services. For more information, please call (216) 201-2000 or visit us on <br />the web at www.ccbh.net. <br />Community and Family Services <br />IMMUNIZATION CLINICS <br />Immunizations for all ages and TB testing are given at CCBH clinics. Travel vaccine clinics are available twice <br />per month. <br />NEWBORN HOME VISITING: WELCOME HOME <br />Mothers that are 200% or greater of the Federal Poverty Level are eligible to receive home visits from a public <br />health nurse. Also eligible for a visit are all teen moms and any mom with an identified medical or social risk <br />factor. The nurse will discuss basic newborn care, home safety, parenting, feeding, and concerns of the <br />parent(s). Additionally, the visit will consist of an assessment of the baby, and will provide the parent(s) with <br />resources and make referrals to community agencies as needed. <br />LEAD POISONING PREVENTION <br />Public health nurses provide lead screening, education, and case management. Public health sanitarians <br />perform investigations in response to reports of elevated blood lead levels in children to identify potential <br />urces of lead exposure. Lead risk assessments, housing remediation, and clearance testing are conducted as <br />part of collaboration with the Cuyahoga County Depa.rtment of Development and our municipalities, through a <br />grant with the U.S. Department of Housing & Urban Development. <br />BUREAU FOR CHILDREN WITH MEDICAL HANDICAPS (BCMH) <br />BCMH is a tax-supported health care program of the Ohio Department of Health. BCMH helps link children <br />with special health care needs and their families to qualified providers. The program assists families in <br />obtaining services to diagnose a possible medical condition (Diagnostic Program) and assists with obtaining <br />payment for ongoing services their child might need (Treatment Program). A child may be eligible from birth - <br />21 years of age. <br />TITLE X FAMILY PLANNING <br />Addresses the gap and provides for direct care in reproductive health and wellness services and education, <br />outreach activities, HIV testing, pregnancy testing and planning, STD testing and treatment, and referral to <br />social services as necessary <br />BREAST AND CERVICAL CANCER PREVENTION PROGRAM (BCCP) <br />This program forms area partnerships with the goal of reducing the mortality of breast and cervical cancer. The <br />program includes screening, case management, tracking, and follow-up as well as public education. <br />DENTAL O.P.T.I.O.N.S. <br />This program links patients with participating dentists or dental clinics where low income patients can receive <br />xe at reduced fees or free of charge.
