HEALTHY HOMES PROGRAM <br />This program investigates the relationship between housing conditions and the respiratory health of occupants <br />living in at risk housing. Public health sanitarians conduct healthy homes investigations for income qualifying <br />residents in homes of infants and seniors at risk for respiratory disease (including asthma). Interventions aze <br />conducted through a grant with the U.S. Department of Housing & Urban Development. <br />INDOOR AIR QUALITY <br />The Indoor Air Quality (IAQ) Program involves the investigation and testing of residences, schools and other <br />public buildings for various indoor air quality indicators on a complaint basis. <br />CREATING HEALTHY COMMUNITIES <br />The Creating Healthy Communities (CHC) Program was created to lower the leading causes of death in our <br />communities. Since many diseases are linked to living habits such as poor diet, low activity levels, and <br />smoking, the CHC program is designed to create system and policy changes that promote healthy eating, active <br />living and smoke-free areas in our schools, work places, health caze facilities, and neighborhoods. The two main <br />areas of concentration for this program are East Cleveland and Euclid, Ohio. <br />PERSONAL RESPONSIBILITY EDUCATION PROGRAM (PREP) <br />The goal of the PREP program is to reduce teen pregnancy and sexually transmitted infection rates for youth in <br />Cuyahoga County and four surrounding counties who are currently in foster care or the juvenile justice systems <br />by educating the direct-care staff in those systems to become trainers in evidence-based pregnancy prevention <br />programming. This train-the-trainer model seeks to enhance professional development at the local level for <br />direct care staff while providing sustainability for pregnancy prevention and life skills education for youth in the <br />foster care and juvenile justice systems. <br />RYAN WHITE <br />The Ryan White HN/AIDS Part A program provides HIV-related services for those who do not have sufficient <br />health caze coverage or financial resources for coping with HN disease. The Cuyahoga County Board of <br />Health (CCBH) serves as the Administrator of the Cleveland Transitional Grant Area (TGA) which serves the <br />following Ohio Counties: Cuyahoga, Ashtabula, Geauga, Lake, Lorain, and Medina. The Ryan white Part A <br />program contracts with local community partners to provide services in the areas of: outpatient ambulatory, <br />medical case management, oral health, medical nutrition therapy, mental health, early intervention, home health <br />care, home and community health care, substance abuse inpatient and outpatient, hospice, housing, medical <br />transportation, outreach, food bank/home delivered meals and psychosocial support services. <br />Community Assessments <br />REPORTABLE DISEASE SURVEILLANCE/PUBLIC HEALTH PREPAREDNESS GRANT <br />The Public Health Emergency Preparedness Grant (PHEP) encompasses reportable disease and surveillance <br />activities. The purpose of this program is to increase public health response and capacity regarding infectious <br />disease, acts of bioterrorism and other major public health events. Activities also include developing emergency <br />plans and supporting the County Emergency Operations Plan by serving as the lead agency under Emergency <br />Support Function (ESF) #8. This program is also supported by the Cities Readiness Initiative (CRI) grant. <br />IMMUNIZATION ACTION PLAN (IAP) <br />This program is designed to raise infant immunization rates in Ohio so that 90% of children will be up to date <br />by 24 months of age. This is accomplished by implementing best practices identified to increase immunization <br />rates including immunization assessment, education and tracking objectives. 100*" <br />2
