R. BIDDER AFFIL,y„ VIT AND CERTIFICATION <br />Each bidder, who is a foreign corporation, that is, a co poration not chartered in Ohio, <br />but licensed to do business in Ohio, is required to submit with his bid an affidavit duly <br />executed by the president, vice president, or general manager of the corporation, stating <br />in said affidavit that said foreign corporation has, in accordance with the provisions of <br />the Revised Code of the State of Ohio, obtained a certificate authorizing it to do <br />business in the State of Ohio. <br />S. VENDOR OR SUPPLIER'S LIABILITY INSUR.ANCE <br />The vendor or supplier shall maintain such insurance as will protect him from claims <br />under Workmen's Compensation acts and other employee benefit acts from claims for <br />damages because of bodily injury, including death to him, employees and all others. <br />The vendor shall provide as listed herein: <br />CONTRACTOR'S PROTECTIVE LIABILITY <br />Bodily Injury $300,000/$500,000 <br />Property Damage $100,000/$300,000 <br />AUTOMOTIVE LIABILITY (Owned, Non-Owned and hired Automobiles) <br />Bodily Injury $300,000/$500,000 <br />Property Damage $100,000 <br />This coverage will be for each occurrence which may arise out of/or result from the <br />vendor's or supplier's operation under the contract, whether such operations be by <br />himself or by any sub-contractor or anyone directly or indirectly, employed by either of <br />them while on the premises of the City of North Olmsted, during the delivery of <br />contract equipment. <br />T. AFFIRMATNE ACTION AND EQUAL EMPLOYMENT <br />The successful vendor, upon request, will be expected to furnish satisfactory proof to <br />the City that they are in full compliance with all applicable Affirmative Action Equal <br />Opportunity Laws, whether Federal, State or of the City of North Olmsted. <br />U. NON-COLLUSION AFFIDAVIT <br />The Non-Collusion Affidavit, which is a part of the bidding documents, shall be <br />properly executed and notarized. <br />V. QUESTIONS <br />It is the responsibility of each bidder, before submitting a bid, to notify the Director of <br />Public Safety of discrepancies, inconsistencies, ambiguities, errors or obvious omissions <br />in the bid documents prior to the submittal of their bid. All questions about the <br />meaning or intent of the bid document are to be directed to the Service Director, or his <br />designee. <br />Corrections, clarifications or interpretations considered necessary by the Director of <br />Public Safety in response to such notifications wiil be issued by addenda mailed or <br />delivered to all parties recorded by the Service Director as having received the bid <br />documents. Notifications received less than seven (7) days prior to the date for opening <br />the bids may not be answered. Only notifications answered by formal written addenda <br />will be binding. <br />5
