R. BIDDER AFFIL,y„ VIT AND CERTIFICATION Each bidder, who is a foreign corporation, that is, a co poration not chartered in Ohio, but licensed to do business in Ohio, is required to submit with his bid an affidavit duly executed by the president, vice president, or general manager of the corporation, stating in said affidavit that said foreign corporation has, in accordance with the provisions of the Revised Code of the State of Ohio, obtained a certificate authorizing it to do business in the State of Ohio. S. VENDOR OR SUPPLIER'S LIABILITY INSUR.ANCE The vendor or supplier shall maintain such insurance as will protect him from claims under Workmen's Compensation acts and other employee benefit acts from claims for damages because of bodily injury, including death to him, employees and all others. The vendor shall provide as listed herein: CONTRACTOR'S PROTECTIVE LIABILITY Bodily Injury $300,000/$500,000 Property Damage $100,000/$300,000 AUTOMOTIVE LIABILITY (Owned, Non-Owned and hired Automobiles) Bodily Injury $300,000/$500,000 Property Damage $100,000 This coverage will be for each occurrence which may arise out of/or result from the vendor's or supplier's operation under the contract, whether such operations be by himself or by any sub-contractor or anyone directly or indirectly, employed by either of them while on the premises of the City of North Olmsted, during the delivery of contract equipment. T. AFFIRMATNE ACTION AND EQUAL EMPLOYMENT The successful vendor, upon request, will be expected to furnish satisfactory proof to the City that they are in full compliance with all applicable Affirmative Action Equal Opportunity Laws, whether Federal, State or of the City of North Olmsted. U. NON-COLLUSION AFFIDAVIT The Non-Collusion Affidavit, which is a part of the bidding documents, shall be properly executed and notarized. V. QUESTIONS It is the responsibility of each bidder, before submitting a bid, to notify the Director of Public Safety of discrepancies, inconsistencies, ambiguities, errors or obvious omissions in the bid documents prior to the submittal of their bid. All questions about the meaning or intent of the bid document are to be directed to the Service Director, or his designee. Corrections, clarifications or interpretations considered necessary by the Director of Public Safety in response to such notifications wiil be issued by addenda mailed or delivered to all parties recorded by the Service Director as having received the bid documents. Notifications received less than seven (7) days prior to the date for opening the bids may not be answered. Only notifications answered by formal written addenda will be binding. 5
