Cleveland State Uliiversity FEDERAL COMMUNITY SERVICE LEARNING PROGRAM AGREEMENT This Agreement is entered into by and between Cleveland State University, hereinafter referred to as the "University", and the C1 ty of North Olmsted, OhlO , located at 5200 Dover Center Road, Nort.h 0?msted. Ohio 44070, hereinafter known as the "Agency", and being a federal, state, or local public agency or political subdivision, or private, non-profit organization. WITNESSETH: WHEREAS, the University is operating a Federal Community Service Learning Program (FCSLP) under the provisions of 42 U.S.C. 2756a and applicable regulations; and WHEREAS, the Agency desires to utilize the services of one or more students participating in the Program. NOW, THEREFORE, it is hereby agreed as follows: 1. The Agency shall provide to the University for review and approval a written job description of each position for which services may be sought. Students shall be assigned only to jobs approved by the University. The Agency shall fumish adequate, qualified supervision of students placed with the Agency. 2. The Agency has the right to control and direct the services of students, not only as to the result to be accomplished but also as to the means by which the result is to be accomplished. The Agency's employment policies shall apply to the extent such policies are consistent with this Agreement. 3. Within thirty (30) days, the Agency shall provide in a tunely and regular manner a detailed Record of Hours Worked and a Billing Statement that shows a certification of hours reported, signed by the Supervisor. This shall include the name of each student, each student's hourly wage rate; the number of hours each student worked for the applicable period; and the total of such earnings during the pay period. Requests for Reiniburseizzent shall be faxed to ? 1 fi/Fi87-y?4'7 or mailed to: Cleveland State University Student Employment Office, UC560 Cleveland State University 2121 Euclid Avenue Cleveland, Ohio 44115 4. The University has the right to review prospective work assignments to determine appropriateness of assignment, determine whether individual students meet the eligibility requirements, refer students to the Agency, and perform site visits as may be practical. 5. The number of students to be placed with the Agency and the hourly rate(s) of pay shall be as mutually agreed between the Agency and the University. The Agency agrees to notify the University and confum the eligibility of each student employee under this Program prior to beginning that student's work assignment under this Program. 6. The Agency acknowledges that students must maintain eligibility to work in this program, that students may not work and the Agency will not be reixnbursed for any hours worked by students during class tunes for which students are registered, and that students may not work and the Agency will not be reimbursed for any hours exceeding 20 hours per week during academic
