12.3 In conjunction with participating within a medical plan, the employee may subscribe <br />to and participate in the following optional health benefit coverage(s). <br />1. Dental <br />2. Vision -Optional Plan #1(Vision plan which was in effect August 1, 1994) <br />Optional Plan #2 (Vision plan similar to that offered throughout Union <br />Eye Care). <br />3. Dental and Vision (Option Plan #1 or #2). <br />Employees participating and subscribing to the optional health benefits listed above <br />within this section, shall contribute an amount equal to that which is listed within Section 12.2 of <br />this Article. Employees must participate in the medical plan to be permitted to participate within <br />the optional benefit coverage listed within this section. The employee contribution shall be <br />withheld as a payroll deduction not later than the first pay period of each month. <br />12.4 Expected cost or premium is defined as the cost of all medical, hospital, prescription <br />drug, and related fees including, but not limited to administrative fees. <br />12.5 The employer reserves the right to establish all plans, enrollment periods and <br />regulations. <br />12.6 The employer shall make an earnest effort to establish a committee consisting of one (1) <br />representative from each of the city bargaining units, if they choose to be represented and a <br />minimum of one 1) representative of the employer. The bargaining unit's role as a participant in <br />the aforementioned committee shall be advisory only. <br />12.7 All changes affecting the plan coverage's co-payments, deductibles, providers, <br />(17) <br />
