14.5. If an employee is absent on sick leave for more than three (3) consecutive work <br />days, the City shall require the employee to present medical evidence that the employee was under <br />a doctor's care and is able to return to work. The City will review the attendance records of <br />employees periodically and at least once in each three (3) month period. If it believes that an <br />employee is developing an attendance problem, it will notify the employee in writing that medical <br />evidence will be required for absences of three (3) days or less. When an employee's record <br />becomes satisfactory, the requirement for such medical evidence for short term absences will be <br />removed, and the employee will be notified in writing. <br />14.6. Any abuse of sick leave or the unjustified patterned use of sick leave shall be just <br />and sufficient cause for discipline, including discharge, as maybe determined by the Employer, <br />subject to the grievance and arbitration procedure. <br />14.7. All employees shall receive two (2) eight (8) hour days o~ with <br />pay, or the Employee may elect to receive sixteen (16) hours pay for each six (6) consecutive <br />months of unused sick leave. Unused sick leave days earned maybe carried over, if nor taken <br />within one (1) year of date of day earned. to the following year, with a maximum of five (5) days. <br />14.8. Accumulated unused sick leave time maybe converted at the time of retirement or <br />death for all fiill-time sworn members of the Police Department. The payment for all those who <br />qualify under this Section shall be based upon the rate of pay of the employee at the time of his <br />retirement or death, and payment shall amount to one-half (1/2) of the unused accumulated sick <br />(20) <br />
