Full-time employees who have completed twenty <br />years through infinity are entitled to six weeks <br />vacation; said vacation shall be taken during <br />the twenty first year and all subsequent years <br />thereafter. <br />u. Any employee may be paid for vacation time in lieu of <br />time off provided, however, that approval therefor is obtained <br />from the employee's immediate supervisor and, in any event, all <br />employees must take a minimum of five uninterrupted days off each <br />year. <br />v. If a day recognized as a Paid Holiday falls within an <br />employee's vacation week, the employee shall have the option to <br />be paid an extra eight (8) hours, at a straight time rate or take <br />the holiday off at another time. <br />SECTION 8: Life Insurance <br />a. After one (1) month of full-time employment, salaried <br />employees shall be covered by an eighteen thousand dollar <br />($18,000) group term life and AD&D policy at employers expense. <br />b. Effective March 1, 1992 the covered amount shall be <br />$20,000. <br />c. To the extent permitted by insurance providers rules, <br />employees may purchase additional life insurance, which shall be <br />deducted from paychecks once a month. <br />d. To the extent permitted by the insurance providers <br />rules, employees may convert to individual policy upon <br />termination of employment or retirement. <br />SECTION 9: Health Insurance <br />a. The Employer shall provide either individual or family <br />medical insurance coverage, at the employees option for each <br />full-time employee. Effective August 1, 1991 through December <br />31, 1991; the Employer shall pay a maximum of $370.67 toward the <br />monthly health premium including riders (dental, vision, drugs). <br />The Employer shall have the option of seeking alternate health <br />insurance providers and will attempt to continue to offer at <br />least two different plans. <br />
