Effective January 1, 1992 the employer shall provide <br />either individual or family medical insurance coverage at the <br />employees option for each full-time employee. The employer shall <br />have the option of seeking alternate health insurance providers <br />or plans and will attempt to offer at least two different plans. <br />Basic hospitalization will be consistent with and similar to <br />those plans offered on December 1, 1991. Basic hospitalization <br />will be that offered to other employees of the City of North <br />Olmsted. The employer has the right to negotiate co-payment and <br />prescription drug rates with providers to lower the cost to the <br />employer as well as the employee contribution. <br />c. PERIOD OF JANUARY 1, 1992 TO JUNE 30, 1992 <br />Effective during January 1, through June 30, 1992, the <br />maximum employer contribution for all medical, hospital, <br />prescription drug and related fees shall be $410.00 per month. A <br />payroll deduction of $30.00 per month shall be made by all <br />employees in plans which have an expected cost or premium in <br />excess of $440.00 per month. <br />If the average cost or premium is established to be <br />less than $440.00 per month over the above six (6) month period, <br />the employee who paid with a payroll deduction contribution shall <br />be entitled to a refund determined by the difference between <br />$440.00 and the actual average cost or premium times six (6) <br />months, but in no event greater than that employee's contribution <br />during the period specified above. Employee's who may be <br />entitled to a refund shall receive such refund within thirty (30) <br />days following the determination of the actual average cost for <br />the period, January 1, 1992 through June 30, 1992. <br />d.
