„, <br />CITY OF NORTH OLMSTED <br />ORDINANCE N0. 94-91 <br />Page 8 <br />If the average cost or premium cost per month exceed <br />$525.00 per month, the additional amount shall be paid at a ratio <br />of three-quarters (75%) by the City and one-quarter (25%) by the <br />employee up to $650.00. If the average cost exceeds $650.01, <br />that additional amount shall be paid by the employer at a ratio <br />of one hundred percent (100%). The employee share of the cost <br />which exceeds an average of $525.,00, shall be withheld as an <br />additional payroll deduction within thirty (30) days following <br />the determination of the actual average cost. <br />f. The employer shall pay the full amount of dental <br />insurance for all full-time employees and dependents. The <br />features of the plan will be consistent and similar to those in <br />effect on December 1, 1991. Some of its features being a $25.00 <br />per person per calendar year deductible together with an annual <br />maximum of one thousand dollars ($1,000) payable per covered <br />person during any benefit year. Those employee's on a family <br />health plan will be covered under a family dental plan. Those on <br />a single health plan will be provided with a single dental plan. <br />g. The employer shall pay the full amount of vision <br />insurance for all full-time employees and dependents if employee <br />chooses a family plan. The features of the plan will be similar <br />and consistent with those in effect December 1, 1991. Those <br />employee's on a family health plan will be covered under a family <br />vision plan. Those on a single health plan will be provided with <br />a single vision plan. <br />h. Expected Cost and Premium is defined as the cost of <br />all medical, hospital, prescription drug cost, and related fees. <br />Retroactivity shall apply to the terms of Sections 9 a, <br />9 b, and 9 c. <br />The employer reserves the right to establish all <br />enrollment periods and regulations. <br />SECTION 10: Sick Leave <br />a. Sick Leave for salaried employees shall continue to be <br />governed by City of North Olmsted's Ordinance Numbers 76-113 and <br />80-50 effective with any revisions to either of the above <br />mentioned Ordinances. <br />
