All other streets without curbing. $ 750.00 <br />901.06 ISSUANCE REQU~~Ni'S. <br />No such permit shall be issued until: <br />(a) The applicant shall have made the deposit referred to in Section 901.01 <br />or 901.07. Such permit shall state the exact time the opening or excavation <br />is to be made, the probable length of time for doing the work and the making <br />of the repair, the character of the pavement, including its base, and the <br />character of the work necessary for the satisfactory repairing or relaying of <br />the roadway. <br />(b) The applicant shall have submitted a policy or policies of liability or <br />indemnity insurance issued by companies licensed to do business in the State, <br />recognizing the City as an insured, in a total amount of not less than five <br />hundred thousand dollars/one million dollars ($500,000/1,000,000) for any <br />personal injury or injuries incurred and two hundred thousand dollars <br />($200,000) for any property damage caused as a result or consequence of the <br />making or existence of such opening or excavation, or the condition of the <br />road, street, sidewalk, tree lawn or other public property in which the same <br />is or has been made. <br />901.03 NOTIFICATION OF POLICE DIVISION. <br />Upon the issuance of a permit set forth in Section 901.02, it shall be the <br />duty of the City Engineer to advise the Police Division of the location, time <br />and duration of the street opening, in order that preparation can be made by <br />the Police Division for traffic control in the area of the street opening. <br />901.04 CONTRACTOR TO MAKE STREET REPAIRS: FAILURE TO COMPLY. <br />(a) Upon completion of the work authorized by the permit granted by <br />compliance with the provisions of this chapter, the contractor shall <br />make such temporary repairs as are necessary to maintain traffic and <br />shall promptly repair permanently the roadway and/or right of way as <br />soon as possible. In the event the contractor fails to make such <br />repairs within ten days after notice from the City, the City may cause <br />such repairs to be made and the cost deducted from the deposits required by <br />Section 901.01. In the event of an emergency or condition causing a serious <br />hazard to the public, the Service Director may cause such hazard to be <br />immediately abated and the cost of such abatement shall be deducted from <br />from the deposits. <br />(b) All repairs shall be done to the satisfaction of the City Engineer. <br />901.05 DEDUCTION AND REFUND. <br />When such work is completed to the satisfaction of the City Engineer, the <br />cost of inspection and supervision shall be forwarded to the Finance Director, who <br />shall deduct from the deposit made by the applicant the cost incurred by the City <br />and shall issue a check to the applicant for the return of the excess over and <br />above the amount necessary to reimburse the City for the cost of inspection and <br />supervision. Where the repair work is done by or under the direction of the City, <br />the cost of such work shall be deducted from the deposit required under Section <br />901.01, together with the cost of engineering and supervision. Any excess <br />remaining, after payment of charges for such work, shall be returned to the <br />applicant as provided above. <br />
