<br />(b) If the applicant is an individual, the name, address, social security number, and date of birth of the
<br />applicant. If the applicant is a club, society, firm, organization, corporation, or group of individuals, the names,
<br />addresses, social security numbers, and dates of birth of the officers, managers and/or members of said organization.
<br />(c) Whether the applicant (or any person listed on the application) has engaged in operating any dance hall
<br />or other teen dance club in the past five years, and when, where and how long in each place.
<br />(d) The name of the owner of the premises in which such teen dance club is to be operated, and the
<br />complete address of such owner.
<br />(e) The names, addresses, social security numbers, dates of birth and signatures of each person who will be
<br />serving as a "Supervising Adult or Chaperon" as required by Section 763.14 below. If the owner of the premises
<br />rents the hall or club to an individual or organization who intends to use same for a teen dance, the owner of the
<br />premises shall notify the Director of Public Safetv of
<br />the fact of the rental and purpose of the event and designate an individual or, in the case of an organization, a
<br />manager or one of its principal officers who shall provide the information required by this section and listed above.
<br />(f) The application shall be signed by the applicant or applicants, or in case of a club, society, firm,
<br />organization or corporation, the application shall be signed by the manager or one of its principal officers. Each
<br />person whose name is listed on the application for license or notification of designee in any capacity shall sign an
<br />authorization for a criminal record check by the City of North Olmsted.
<br />(g) A completed Ohio Bureau of Criminal Identification and Investigation (BCI) background check
<br />obtained at applicant's cost shall accompany the application The BCI background check must be of the
<br />applicant, and anv Adult Supervisor or Chaperon if named in the application if the application is for an
<br />individual, and must be for all officers, managers and/or members and anv Adult Supervisor or Chaperon
<br />named in the application, if the applicant is a club society firm organization corporation or other grout of
<br />individuals.
<br />763.08 INVESTIGATION OF LICENSE APPLICATION.
<br />The ~''~~~ ''mow Director of Public Safetv shall require the Chief of Police to conduct, or assign to a
<br />member of the Police Division to conduct, an investigation of each application for a teen dance club license and
<br />determine whether the application complies with all of the regulations, ordinances and laws applicable thereto, and
<br />shall cause an investigation as to the character and fitness of the applicant or applicants, or the officers of the club,
<br />society, firm, organization or corporation (including each person listed on the application) and of the person, who is
<br />to have the general management of such teen dance club, as well as an investigation of all persons listed
<br />as "Supervising Adults or Chaperons" for teen dances.
<br />763.09 LICENSE APPLICATION DENIALS.
<br />The application for a teen dance club license shall be denied by the Director of Public Safety if the report
<br />submitted therewith shows that any of the persons named in the application (including Supervising Adults for teen
<br />dances) have previously been connected with a public dance hall or a teen dance club where the license has been
<br />revoked, or where the place sought to be licensed has been the source of repeated incidents or a pattern of disorderly
<br />or criminal conduct, or has otherwise substantially interfered with public peace or good order in the neighborhood,
<br />or where any of the provisions with reference to public dance halls or teen dance clubs have been violated, or if the
<br />place sought to be licensed as a teen dance club does not comply in every way with the regulations, ordinances and
<br />laws applicable thereto. In addition, if it is determined that any person named in an application (er including any
<br />Adult Supervisor for teen dances) has been convicted within the last ten years of a~felony, or anv misdemeanor
<br />drug or alcohol offense, corruption of a minor or child endangering offense, domestic violence or assault offense,
<br />or ether anv crime of moral turpitude, •°~*'~~~ ~h° ~^^« ~ ^ -~°^-^, the license shall be denied.
