<br />(e) an establishment that: <br />(1) has a restaurant certificate of occupancy; <br />(2) has a valid food and beverage certificate; <br />.~•, <br />(3) derives 50 percent or more of its gross revenues on a quarterly basis from the sale of food <br />and nonalcoholic beverages; and <br />(4) does not charge an entrance or admission fee; and <br />(f) an establishment where: <br />(1) persons contract for instruction in dance methods, styles, techniques, and disciplines <br />recognized by professional dance organizations; <br />(2) the dance instruction is only provided by persons trained in dance methods, styles, <br />techniques, and disciplines recognized by professional dance organizations; and <br />(3) no dancing occurs on the premises except by: <br />(A) a person employed or under contract to provide dance instruction at the <br />establishment; and <br />(B) a person contracting for dance instruction at the establishment; and <br />(4) the primary purpose of the dancing at the establishment is for the education of the <br />persons contracting for dance instruction and not for the entertainment of the <br />general public. <br />763.06 APPLICATION FOR LICENSE; RECEIPT AND POSTING. <br />Every person, society, club, firm, organization, corporation, or other legal entity, desiring a license to <br />operate a teen dance club must make personal application to the ~;I~a€e€~el~e Director of Public Safety for a <br />license. Each application shall contain the information set forth below in Section 763.07 and shall be on a form <br />prescribed by the Director . <br />The Director shall issue the license within twenty days following the date the <br />application is received, unless cause exists as provided in Section 763.09 below to deny the license. Upon <br />receiving a license to operate a teen dance club, the owner or operator shall post the license in a conspicuous place at <br />the teen dance club. <br />763.07 INFORMATION REQUIRED ON APPLICATION. <br />Every applicant for a teen dance club license shall submit a written application stating the name and <br />complete address, including nine digit zip code , of the applicant, or if more than one person or if <br />an association or firm, the full name of all parties interested and the complete addresses including nine digit zip <br />code °*~°°' ~^a ~~~^~~-°- If the applicant is a club, firm, society, organization or corporation, the applicant shall <br />contain a complete list of the officers of such club, society, organization, firm, or corporation, with their names and <br />addresses, including nine digit zip code ,and shall also give the state in which such club, society, <br />organization, firm or corporation is organized, its federal employer identification number (EIN) and the names <br />of one or more persons whom the club, society, firm, organization or corporation desires to designate as its manager <br />or managers, or person or persons, in charge with their complete addresses. The application shall also state the <br />following: <br />(a) The location where such teen dance club is to be operated, specifically, complete address and nine <br />digit zip code for said location ' <br />
