Each death in the immediate family, a maximum of five <br />(5) eight (8) hour work days, including travel time. <br />The term "immediate family" in the case of death shall <br />be interpreted as meaning, parents, parents of spouse, <br />spouse, and children of the employee, regardless of place <br />or residence, as well as other relatives living in the <br />employee's home. <br />4. Each death of, brother or sister of employee, or spouse, <br />five (5) work days, including necessary travel time. <br />C. Proof of Absence <br />The Department Head shall require that the employee furnish a <br />satisfactory affidavit to the effect that the absence was caused <br />by illness, or due to any of the foregoing causes. <br />D. Return to Full Time Duty. <br />Employees who have been absent for three (3) days or more, due <br />to illness, shall be required to present a statement from a <br />physician validating that they are physically able to resume <br />their duties on a full time basis. <br />Vdhen an employee has been absent for less than three (3) days, <br />the Department Head, at his discretion, may request a statement <br />from the employee or his physician before the employee may <br />return to work. <br />At the discretion of the Department Head, Director, or Mayor, <br />the City Physician may be requested to call upon any City <br />employee. <br />In the event any employee has been off duty for a serious ailment, <br />such as a heart attack, or any employee has suffered loss of limb. <br />sight, physical or mental disability, and if any such employee <br />is unable to perform the duties he or she performed prior to <br />his or her disability, the Director of Mayor may deem it <br />necessary to have the employee approved by the City Physician <br />before said employee goes back to work. Decision, as to whether <br />the employee is capable of fulfilling his or her duties, shall <br />be at the discretion of the Director of Public Service or the <br />Mayor, but if the approval to return to work is not given, as <br />set forth above, the employee may request the City Physician for <br />an examination, at his or her cost, and the Mayor and the City <br />Physician shall issue a joint approval or disapproval, whichever <br />the case may be. <br />E. "Sick Leave" is to be accumulative by the employee and is only <br />to be paid upon furnishing documented medical certification of his <br />or her illness. <br />"Sick Leave" Is Not To Be Used For Retirement Purposes, As For <br />Example: <br />An Earlier Retirement. <br />
