ARTICLE XII HEALTH INSURANCE <br />12.01 The Employer shall provide, pursuant to Appendix A; either individual or family medical <br />insurance coverage at the employee's option for each full-time employee. The insurance <br />coverage in effect in 2009 shall continue through 2011. The Employer shall have the option of <br />seeking alternate health insurance providers or plans. <br />12.02 Effective January 1, 2008, employees shall pay ten (10%) percent and the Employer shall <br />pay ninety (90%} percent of the insurance plan costs, <br />All employee contributions, as required by this Article, shall be withheld via payroll <br />deduction not later than the first pay period each month for all the employee percentage <br />contributions stated above. <br />12.03 Employees shall participate in the AFSCME Benefit Plan at the present level of benefits. <br />Vision - Optional Plan #1 (Vision plan which was in effect August 1,1994). <br />Optional Plan #2 (Vision plan similar to that offered tluoughout <br />Union Eye Care}. <br />12.04 Expected cost or premium is defined as the cost of all medical, hospital, prescription <br />drug, and related fees including, but not limited to administrative fees. <br />12.05 The Employer reserves the right to establish all plans, enrollment periods and regulations. <br />12.06 All changes affecting the plan providers, enrollment periods and regulations shall be <br />considered subject to recommendation by the healthcare committee in accordance with Section <br />12.09. <br />12.07 The Employer will secure a commercial insurance policy whereby any bargaining unit <br />member killed in the line of duty shall receive to his spouse or estate, a payment of fifty <br />thousand ($50,000.00) dollars. Such payment will be made in accordance with the terms of the <br />commercial plan. <br />12.08 The Employer shall cause the Health Care Plan to be a qualified Plan under Section 125 <br />of the Internal Revenue Code. <br />12.09 Effective January 1, 2008, a new health care advisory team (HCAT) shall be formed and <br />shall meet for the purpose of creating or revising the current health care coverage/plans for <br />employees of the City. The HCAT shall consist of fourteen (14) employees who must be <br />selected from the following employee groups: <br />One (1) employee from Corrections, One (1) employee from Dispatch, Two (2) <br />employees from Clerical/Technical, Two (2) employees from Service, two (2) employees <br />from Fire, Two (2) employees from Police, Two (2) non bargaining unit employees, and <br />Two (2) employees from Administration. The Employer shall provide advisors of <br />facilitators to assist the HCAT regarding health care. <br />The HCAT shall be required to review the City's cun•ent health care plans for medical, <br />dental, and vision benefits, and shall recommend to the City Council new or revised plans <br />
