time can be taken as needed by the employee, for example, one (1) or two (2) days at a time. An <br />employee will give the supervisor at least forty-eight (48) hours advance notice in writing. In an <br />emergency situation, verbal communication with the supervisor is appropriate and may justify <br />waiving advance notification. <br />10.08 The vacation sell back limit of one (1) week per employee shall expire and be of no effect <br />on January 1, 2012 for the Union (FOP). <br />ARTICLE 11 PROBATIONARY PERIOD <br />11.01 The probationary period for all new employees shall continue for twelve (12) months, <br />begimling on the date of hire. During this probationary period, the employee shall have no rights <br />to appeal any form of disciplinary action, including termination through the Grievance Procedure <br />or to any Civil Service Commission. <br />ARTICLE 12 HEALTH INSURANCE <br />12.01 The Employer shall provide, pursuant to Appendix A, either individual or family medical <br />insurance coverage at the employee's option for each full-time employee. The insurance <br />coverage in effect in 2009 shall continue through 2011. The Employer shall have the option of <br />seeking alternate health insurance providers or plans. <br />12.02 Effective January 1, 2008, employees shall pay ten (10%) percent and the Employer <br />shall pay ninety (90%) percent of the insurance plan costs. <br />All employee contributions, as required by this Article, shall be witl~l-leld via payroll <br />deduction not later than the first pay period each month for all the employee percentage <br />contributions stated above. <br />12.03 Employees shall participate in the AFSCME Benefit Plan at the present level of benefits. <br />1. Vision -Optional Plan #1 (Vision plan which was in effect August 1, 1994) <br />Optional Plan #2 (Vision plan similar to that offered tlu•ou~hout Ullloll Eye <br />Care). <br />12.04 Expected cost or premium is defined as the cost of all medical, hospital, prescription <br />drug, and related fees including, but not limited to administrative fees. <br />12.05 The Employer reserves the right to establish all plans, em•ollment periods and regulations. <br />ARTICLE 13 CLOTHING ALLOWANCE <br />13.01 Effective January 1, 2008, all full-time bargaining unit members shall receive an annual <br />clothing allowance of seven hundred twenty-five dollars ($725.00), payable with the first <br />regular pay check in January for the duration of this Agreement. (Clothing allowance shall be <br />pro-rated per month in the employee's final year of employment.) <br />
