the terminafion of the sick leave, Any dispute as to the ability of an employee to perform the <br />work of the job shall be determined pursuant to the medical examination procedure in the <br />Generai Leave Provisions; Section 5. <br />Secfion 3. Paid Sick Leave: An employee shall be granted sick leave for personal iilness <br />or injury as foilows: <br />(a) Paid sick leave shall be earned and accumulated at the rate of <br />one and one fourth (1114) days for each month <br />(b) Pay for sick leave shall be at the employee's straight time hourly rate (or portion thereof if absent for <br />fess than a full day); <br />(c) If an employee is absent on sick leave for more than three (3) consecutive work days, the City shall <br />require the employee to present medical evidence that fhe employee was under a doctors care and is <br />abie to return to work. The City will review the attendance records of empioyees periodically, and at <br />least once in each fhree (3) month periotl. If it believes that an emplayee is developing an attendance <br />problem, it wiN notify the employee in writing fhat medical evidence will be required for absences of <br />three (3) days or less. When an employee's record becomes safisfacfory, the requirement for such <br />medicai evidence for short ferm absences will be removed, and the employee will be notified in writing. <br />(d) Emplayees may use up to fifteen (15) work days per year of earned and accumulated sick leave if <br />necessitated by illness in the employee's immediate family. For purposes of this subsecfion, <br />"immediate family" shall be interpreted to be spouse; children; parenfs of the employee, regardless of <br />the piace of residence; and other relafives living wifhin the household of the empioyee, <br />(e) When an employee with five (5) or more years of service refires, having received PERS pension, dies <br />or fakes disability retirement and has accumulated sick feave fime, the retiring employee may convert <br />one-half (1l2) of the accumulatetl sick leave time (with the City of Norfh Olmsfed) to pay on fhe basis.of <br />his rate of pay at the time of refirement, but the maximum amount paid shafl not exceed one thousand <br />(1000) hours. Employees hired affer Jufy 1, 1998, and have accumulafed sick leave wifh anofher <br />political subdivision will transfer saitl sick leave and shail use the transferred sick leave prior to sick <br />leave accumulafied while employed with the Cify of North Olmsted, Sick leave carry over from another <br />political subdivision shall not be used to ca(culate an employees buy-out pursuant to the above. <br />(fl An employee should make an efforf to schedule medical and dental appaintments for himself and his <br />family members during <br />non-working hours. <br />(g) Sick leave taken the day prior to or fhe day affer a scheduled vacafion or hoiiday will require a medical <br />excuse prior to approval for payment, <br />17
