" Bidder <br />Specifications Complies <br />Yes No <br />AFFIMATIVE AC'TION AND EQUAL EMPL,OYMENT <br />The successful bidder upon request will be expected to furnish satisfactory proof to the city <br />that they are in full compliance with all applicable Affirmative Action Equal Opportunity <br />Laws, whether Federai, State or the City of North Olmsted. <br />MEHTOD OF MAKING AWARD <br />The tota] cost, quality, type of equipment, capabilities and reliability, available training <br />support, quality of available technical sen-ice support, th-- degree and qualitv of sales and <br />administrative support a?,ailable, and warranties included, will be considered in detennining <br />which proposal is most advantageous to the city and there the lowest, best, and most responsive bid. <br />DATE OF AWARI) <br />The cit?, will award the contract or reject all bids within 90 days following the opening of <br />bids. <br />BID BOND <br />Each bid shall be accompanied by a bid bond executed by a surety company authorized to do <br />business in Ohio, or by a certified check on a solvent bank, payable to the City of North <br />Olmsted, in a sum equal to ten (10) percent of the amount of the bid. The certified check or <br />bond will be held as a guarantee that if the proposed bid is accepted the bidder will enter into <br />a contract for same. <br />If a bond is submitited, it shall be issued by a bonding company licensed to bond in the State <br />of Ohio. <br />PERFORMANCE BONB <br />The successful bidder shall be required to submit a performance bond in the amouizt of 100% <br />of the total contract price. Such bond shall be provided within ten (10) days after receiving <br />notice of the award of a contract. Such bonds shall indemnifV the city against the damaaes <br />that may be suffered bv the bidder's failure to perfornl his/her contract accordina to the <br />provisions thereof and in accordance with the contract specifications for this award. <br />FIl\ANCIAL STATEMENT <br />It is the intention of the purchaser to contract for eonstruction of this vehicle with a <br />ilianufacturer who is financially sound. In order for us to evaluate the financial stability of <br />each bidder, a current Financial Statement and -kiinual Report shall be provided 'A'ith the <br />proposal. <br />PRELIMINARY DRAWING <br />A preliminarv drawing shall be submitted with the bidders proposal showina three vie"A7s, <br />dimensions and layout of the proposed apparatus. <br />DIAGRAM MULTIPLEXED ELECTRICAL CIRCUITS <br />Three (3) sets of diagrams of the body and chassis multiplexed DC electrical system shall be <br />Page 5
