Musial, Mayor <br />Mr. Joseph Tamulewicz <br />JAT Associates <br />19510 Westwood-Drive <br />Strongsville, Ohio 44136 <br />Re: Request for Proposal, Design Serviees for ADA Improvements <br />at the North Olmsted Recreation Center, 26000 L,orain Road <br />Dear Mr.' Tamulewicz: <br />I am requesting a Proposal to provide professional design services for ADA improve- <br />ments at the North Olmsted Recreation Center in the City of North Olmsted: The <br />proposed work would include tlie following: <br />l: The City needs to retro-fit the front vestibule entrance area with two (2) doors that are <br />activated for handicapped accessibility. These doors may be a ready-fold/in=fold <br />version and motion sensor activated. They musf provide for ADA requirements and <br />maintain the safety and energy efficiency of'the vestibule area. <br />2. The main lobby restraoms also need to be redesigned and modifed to accommodate <br />a disabled user. This effort may require substantial reconsiruction. <br />The proposal shall include the following items in the. Scope of Seivices: <br />1. General Conditions: <br />(a) All construction documents shall bear the seal of a design professional <br />registered in the state of Ohio (e.g., landscape axchitect, architect, engineer or <br />surveyor, as required by law or regulation for the type of document). <br />(b) Work shall not be subcoiitracted without prior consent of the City in writing. <br />(c) Additional billable services sha11 not be performed without prior consent of the <br />City in writing. <br />e? <br />?«? ??Together We Can Make A Difference! ?? ?
