:~.. <br />provide certificates evidencing such insurance, including coverage for contractual liability, <br />to the CITY in a form acceptable to the CITY. <br />11.2 The policies or certificates for general and motor vehicle liability insurance <br />shall name the CITY as an Additional Insured. <br />11.3 All polices, or certificates therefore, shall provide that 30 days prior to <br />cancellation or material change in the policies, notice of same shall be given to the CITY <br />by certified mail, return receipt requested, for all policies so affected. All notices shall <br />name the CONTRACTOR and identify this contract. Failure to give such notice for any <br />reason shall be a breach of this Agreement which may, at the option of the CITY, cause <br />this Agreement to be declared null and void. <br />11.4 The minimum required coverage is the following: <br />a. Workmen's Compensation <br />Workmen's Compensation and Employer's Liability in compliance <br />with the Statutory requirements of the State of Ohio. <br />b. General Liability Insurance <br />Comprehensive general liability insurance covering operations, <br />completed operations, contractual agreements, and independent contractors, each with <br />minimum limits of liability as set forth below, issued to and covering the liability of the <br />CONTRACTOR with respect to all work performed by the CONTRACTOR under this <br />Agreement. <br />Bodily Injury, including death: <br />$1,000,000 each person <br />$1,000,000 each accident <br />Property Damage: <br />$ 500,000 each accident <br />The provisions of Section 12 Indemnification, shall be included in <br />this policy by separate endorsement. <br />c. Motor Vehicle Liability Insurance <br />Motor vehicle liability insurance including all owned, hired or non- <br />owned vehicle equipment for minimum limits of <br />Bodily Injury, including death: <br />$1,000,000 each person <br />$1,000,000 each accident <br /> <br />
