Dental <br />2. Vision = Optiaual Plan # 1 tVision plan which wae fa <br />effect August 1994.2 <br />Visf.on - Optional Plan #2 (Vision plan simtlar to that <br />offered throuqh Uni,on Eye Care) <br />3. Dental and Vision (Optional Plan #1 or #2) <br />Fu21-time employees participating and subscribing to the <br />optional health- benefits listed above within this section shall <br />contribute an amount equal to that which is listed within Sectian <br />2, (A) and (B) of this Article. Employees shall be required to <br />participate in the medical plan to be permitted to participate <br />within the optionai benefit coverage listed within this Section. <br />The employee contribution shall be withheld as a gayroll <br />deduction not I.ater than the first pay periad of each month. <br />SECTION 4: Expected cost or premium is defined as the cost of <br />all medical, hospital, prescription drug and related fees <br />including but not ].imited to administration fees. <br />SECTION 5: The Eraployer reserves the right to establish a11 <br />plans, enrollment periods and regulations. <br />SECTION 6: The Employer shall make an earnest effart to <br />establish a committee consisting of one (1) representative from <br />each of the City bargaining units, if they choose to be <br />represented and a minimum of one (1) representative of the <br />Employer.
