<br />1.00 Sc91'E <br />Subsection 101, Flows Accented by North Olmsted, is revised as follows: <br />(1) In paragraph 1.01 (a) the first sentence is revised in its entirety as follows: "The rate of <br />flow of wastewater from the Service Area into the System shall not exceed gallons <br />per minute at any time in the Esther Avenue sanitary sewer gallons per minute at <br />any time in the Mastick Road Sanitary Sewer." <br />(2) In paragraph 1.01 (b) the first sentence is revised to read in its entirety as follows: "The <br />average aggregate flow of wastewater from the Service Area into the system, computed <br />on an annual basis, shall not excced 0.475 million gallons per day". <br />1.02 Flow Meters ()oerated by North OUnsted <br />Subsection 1.02 is revised by inserting the following paragraph between the exisdng first <br />and second paragraphs: <br />"If required by subsection 2.01, Fairview Park shall install and North Olmsted shall operate and <br />maintain a master flow meter at the Mastick Road point of entry to measure the wastewater flow <br />from the Service Area at that point of entry. Upon installation of this master flow meter, all <br />provisions in this subsection pertaining to subsequent measurements of flow at this point of entry <br />shall be based upon the new meter, and the measurement procedures for a temporary flow meter <br />installed by North Olmsted for one month of the year shall be superseded." <br />1.03 CoLlection, Opaation sind ainte 9nce by Fa'rview rk <br />Subsection 1.03 is revised by adding the following paragraph to the end of this subsection: <br />"Fairview Park shall operate and maintain its sewerage facilities for the Service Area consistent <br />with the requirements of the Ohio Environmental Protection Agency Authorization to Dischazge <br />Under the National Pollutant Discharge Elimination System, Ohio EPA Permit No. <br />3PD00016*GD issued to the City of North Olmsted on September 26, 1995, or any subsequent <br />amendments thereto. Specifically, Fairview Pazk shall comply with Part I, C-SCHEDULE OF <br />COMPLIANCE, Item No. 4 for all requirements pertaining to the appropriate measures and <br />methods for the operation of a satellite separate sanitary system. These requirements include but <br />may not be limited to the Best Management Pracdce Plan which sha11 include a schedule for <br />cleaning, inspection and maintenance of all sewerage facilities and the installation of telemetry <br />from lift stations in the Service Area to the Wastewater Trea.tment Plant which shall inelude <br />instantaneous flow rate and instantaneous wet well level. <br />"The telemetry system must be approved by the North Olmsted City Engineer. Fairview Park <br />shall be responsible for the maintenance of the telemetry at and within the lift stations and the <br />devices used to transmit the instantaneous flow rate and instantaneous wet well levels to the <br />Wastewater Treatment Plant.
