ARTICLE 12 <br />HEALTHINSURANCE <br />12.1 The employer shall provide either individual or family medical insurance <br />coverage at the employee's option for each full-time employee. The empioyer shall have <br />the option of seeking alternate health insurance providers or plans and will attempt to <br />offer at least two different plans. Basic hospitalization will be consistent with and similar <br />to those plans offered on 7uly 1, 1994. Basic hospitalization will be that offered to other <br />employees of the City of North Olmsted. The employer has the right to negotiate ca- <br />payment and prescription drug rates with providers to lower the cost to the employeras <br />well as the employee contribution. <br />12.2 Effective January 1, 1998 through June 30, 1998 full-time employees <br />participating within either an individual or family plan shall contribute an amount equal <br />to eight percent (8%) of the plan premium or the expected cost of such medical plan and <br />the employer shall contribute ninety two percent (92%) of such cost. <br />Effective July 1, 1998 through Decetnber 31,1999, full-time employee(s) <br />participating within either an individual or family plan shall contribute an amount equal <br />to ten percent (10%) of the plan premium or the expected cost of such medical plan and <br />the employer shall contribute ninety percent (90%) of such cost. <br />Effective January 1, 2000 through the duration of ttus agreement, full-time <br />employee(s) participating within either an individual or family plan shall contribute an <br />amount equal to twelve percent (12%) of the plan premium or the expected cost of such <br />medical plan and the employer shall contribute eighty pem,ent (88%) of such cost. <br />(16) <br />
