All employee contributions, as required by this Article, shall be withheld via <br />payroll deduction not later than the first pay period each month for all the employee <br />percentage contributions stated above. <br />12.3 In conjunction with participating within a medical plan, the employee may <br />subscribe to and participate in the following optional health benefit coverage(s). <br />1. Dental <br />2. Vision - Optional Plan #1 (Vision plan which was in effect August 1, 1994) <br />Optional Plan #2 ( Vision plan similaz to that offered throughout Union <br />Eye Care). <br />3. Dental and Vision (Option Plan #1 or #2). <br />Employee.s participating and subscribing to the optional health benefits listed <br />above within this section, shall contribute an amount equal to that which is listed within <br />Section 12.2 of this Article. Employees must participate in the medical plan to be <br />permitted to participate within the optional benefit coverage listed within this section. <br />T'he employee contribution shall be withheld as a payroll deduction not later than the first <br />pay period of each month. <br />12.4 Expected cost or premium is defined as the cost of all medical, hospital, <br />prescription drug, and related fees including, but not limited to administrative fee.s. <br />12.5 The employer reserves the right to establish all plans, enrollment periods and <br />regulations. <br />12.6 The employer shall make an earnest effort to establish a committee <br />consisting of one ( 1) representa.tive from each of the city bargaining units, if they choose <br />to be represented and a minimum of one (1) represeataxive of the employer.
