A. Disclosure of name, address and location of the <br />discharger." <br />Be and the same is hereby amended and as amended sfiall read <br />as follows: <br />"(2) Disclosure forms. Industrial dischargers shall complete <br />and file with the City a disclosure declaration in the form <br />prescribed by the City. Existing industrial dischargers shall <br />file disclosure forms within thirty days after the effective <br />date of this section, and proposed new source dischargers <br />shall file disclosure forms at least ninety days prior to <br />connecting to the sewage disposal system. The disclosure <br />to be made by the discharger shall be made on written forms <br />provided by the City and shall cover: <br />A. Disclosure of name, address and location of the <br />discharger." <br />SECTION 5: Section 917.05 (a) (2) C. of the Codified Ordinances <br />of the City of North Olmsted, Ohio which now reads as follows: <br />"C. Disclosure of wastewater constituents and characteristics <br />including but not limited to those mentioned in this chapter, <br />including Appendices A, B and C as appropriate, as determined <br />by bona fide chemical and biological analyses. Sampling <br />and analysis shall be performed in accordance with procedures <br />established by the U.S. EPA and contained in 40 CFR, Part 136, <br />as amended: <br />Be and the same is hereby amended and as amended shall read <br />as follows: <br />"C. Disclosure of wastewater constituents and characteristics <br />including but not limited to those mentioned in this chapter, <br />including Appendices A, B and C as appropriate, as determined <br />by bona fide chemical and biological analyses. Sampling <br />and analysis shall be performed in accordance with procedures <br />established by the U.S. EPA and contained in 40 CFR, Part 136 <br />does not include sampling analytical tecfiniques for the pollut- <br />ants in question, or where the City determines that the Part 136 <br />sampling or analytical techniques are inappropriate for the <br />pollutant in question, sampling and analyses shall be performed <br />using validated analytical methods or any other sampling and <br />analytical procedures approved by the City. If the discharger <br />monitors a pollutant more frequently than required by the City, <br />using the procedures described in this Codified Ordinances, the <br />results of this monitoring shall be included in the report." <br />SECTION 6: Section 917.05 (c) (1) of the Codified Ordinances <br />of the City of North Olmsted, Ohio which now reads as follows: <br />"(c) Re ortin Re uirements for Dischar er. <br />(1) Comp iance date report. Within ninety days following <br />for final compoiance by a discharger with applicable <br />pretreatment standards set forth in the Codified Ordinances <br />or ninety days following commencement of the introduction <br />-4-
