<br />Fees shall be the total cost per year per industry incurred <br />by the Service Director, City Engineer, Wastewater Treatment <br />Plant Superintendent, secretary/word processor plus miscell- <br />aneous costs for office equipment, supplies, paper, copying <br />and transporatation." <br />Be and the same is hereby amended and as amended shall read <br />as followst' <br />11(4) Fees for the City's Administation cost associated with <br />industries required to maintain a scheduled sampling and <br />analysis program, as warranted in the City's industrial <br />pretreatment program. Fees shall be the toatal cost per <br />year for industry incurred by the Service Director, City <br />Engineer, Wastewater Treatment Plant Superintendent, Pre- <br />treatment Coordinator plus misclleaneous costs for office <br />equipment, supplies, paper, copying and transporatation," <br />SECTIQ'N 3: Section 917.05 of the Codified Ordinan.ces of <br />the City of North Olmsted, Ohio which now reads in part as <br />follows: <br />"917.05 WASTEWATER DISCHARGERS. <br />No person shall discharge sewage, industrial wastes or other <br />wastes to any sewer outlet within the jurisdiction of the City, <br />without having first complied with the terms of the Codified <br />Ordinances." <br />Be and the same is hereby amended and as amended shall read as <br />follows: <br />"917.05 WASTEWATER DISCHARGERS. <br />No person shall discharge sewage, industrial wastes or other <br />wastes to any sewer outlet within the jurisdiction of the <br />City, without having first complied with the terms of the <br />Codified Ordinances. <br />New Sources are required to install and have it operating <br />condition, and shall "start up" all pollution control equip- <br />ment required to meet applicable Pretreatment Standards before <br />beginning to discharge. New Sources shall meet all applicable <br />Pretreatment Standards within ninety (90) days of start up." <br />SECTION 4: Section 917.05(a) (2) of the Codified Ordinances <br />of the City of North Olmsted, Ohio which now reads in part as <br />follows: <br />11(2) Disclosure forms. Industrial dischargers shall c.omplete <br />and file with the City a disclosure declaration in the form <br />prescribed by the City. Existing industrial dischargers shall <br />file disclosure forms within thirty days after the effective <br />date of this section, and proposed new dischargers shall file <br />disclosure forms at least ninety days prior to connecting to <br />the sewage disposal system. The disclosure to be made by the <br />discharger shall be made on written forms provided by the <br />City and shall cover: <br />-3-
