<br />SICK LEAVE <br />ARTICLE XXI <br />Section 1. An employee who has completed his probationary <br />period shall be granted sick leave for personal illness or injury <br />as follows: <br />a. Paid sick leave shall be earned and accumulated at the <br />rate of one and one fourth (1-1/4) days for each month; <br />b. Pay for sick leave shall be at the employee's straight <br />time hourly rate (or portion thereof if absent for less <br />than a full day); <br />c. If an employee is absent on sick leave for more than <br />three (3) consecutive work days, the City shall require <br />the employee to present medical evidence that the <br />employee was under a doctor's care and is able to <br />return to work. The City will review the attendance <br />records of employees periodically and at least once in <br />each three (3) month period. If it believes that an <br />employee is developinq an attendance problem, it will <br />notify the employee in writinq that medical evidence <br />will be required for absences of three (3) days or <br />less. When an employee's record becomes satisfactory, <br />the requirement for such medical evidence for short <br />term absences will be removed, and the employee will be <br />notified in writing. <br />d. E?aployees may use up to fifteen (15) work days per year <br />of earned and accumulated sick leave if necessitated by <br />illness in the employee's immediate family. For <br />purposes of this subsection, "immediate family" shall <br />be interpreted to be spouse, children, parents of the <br />employee, regardless of the place of residence; and <br />other relatives living within the household of the <br />employee. <br />e. When an employee with five (5) or more years of service <br />retires, having received P.E.R.S. pension and havinq <br />accumulated sick leave time, the retiring employee may <br />convert one-half (1/2) of the accumulated sick leave <br />time to pay on the basis of his rate of pay at the time <br />of retirement, but the maximum amount paid shall not <br />exceed three hundred and sixty (360) hours paid. <br />f.
