-_1CLIENT/ EMPLOYEE UkINE DRUG SCREEN PROCEDURE <br />.. <br />@Purpose : To maintain strict confidentiality and to obtain <br />optimal collection of urine specimen. <br />Substance Abuse Panel: <br />A urine specimen will be screened for the <br />detection of the following substances: <br />NAMES SCREEN CUT OFF <br />Amphetamines 1000 <br />Benzodiazepine 300 <br />Methaqualone 750 <br />Phencyclidine 25 <br />Barbiturates 200 <br />Cannabinoid SO <br />. Opiates 300 <br />Cocain (Metab) 300 <br />Procedure: <br />1. When examinee arrives, request that he or she present <br />positive photo identification (Driver's License preferred). <br />ii c3iPnt daes not have appropriate identification, contact <br />employer to receive instructions to either continue or stop <br />the collection process. When client presents photo <br />identification, it is pnotocopied onto the consent form. <br />2. The client is advised that a urine specimen will be <br />collected for the purpose of detecting the presence of drugs <br />(if not previously instructed). <br />3. The client is then taken into the examination room, <br />asked to remove clothing and given a hospital gown to put <br />on. If having a physical exam, it will be completed at this <br />time. The client is then given a sterile urine container <br />with a screw top lid and asked to void at least 50cc of <br />urine into the provided container (Lab will accept a minimum <br />of 20cc to forward for testing). <br />4. Tt-ie client is then escorted to the restroom provided <br />(sink taps are pseviously taped with "Confidenxial" security <br />tape and the toilet and tank water are colored). <br />5. Client is instructed to collect specimen, not to flush <br />the toilet or wash his/her hands, to bring specimen back to <br />the examination room and give directly to collector, keeping <br />the specimen in sight at all times. Client may then wash <br />his/her hands. <br />6. In the presence of the client, the temperature (should <br />be within 32.5-37.7°), Ph (5-8), and specific gravity <br />(1.003-1.035) are checked. This is to be done within four <br />minutes of collection. If specimen standards are not met, <br />another specimen is collected and both are sent to <br />Toxicology. This should be noted on the Chain of Custody <br />form. If it is necessary to collect additional samples, <br />separate Toxicology requisitions with Chain of Custody forms <br />should be filled out for each specimen collected.
