In the presence of the client, the container is securely closed <br />with the screw top lid provided. A label with the client's name or <br />.Tane/John Doe an3 Social Security number is placed on the container <br />after verifying the information with the client. The identification <br />label on the Chain of Custody requisition is peeled off and placed <br />on the specimen brottle also. The lid is then secured with the <br />"Confidential" tamper proof red tape. This tape is initialled on the <br />top and the side by the client. <br />8. The specimen-is then placed in the plastic.bag provided for <br />Chain of Custody handling. <br />9- The Chain of Custody form is completed and the client's name or <br />Jane/John Doe, Social Security number, collection date and .time are <br />checked for.accuracy. The donor may sign on the designated line <br />(employees may initial since only the Social Security number is . <br />used to insure confidentiality). , <br />10. The collector <br />has recently taken <br />right upper comer <br />appropriate transpi <br />line (signature of <br />the date. <br />lists all medications the donor is taking or <br />(prescription or over the counter) in the <br />-of the Chain of Custody form, checks the <br />3rt method (RBL courier), signs the designated <br />attendant at collection site),-and-completes <br />11. The original copy of the Chain of Custody form• zs placed in <br />the Chain of Custody bag with the urine specimen and sealed <br />appropriately. The remaining copies of the Chain of Custody formy <br />are placed in the provi.ded area on the back of the bag, <br />12. The specimen is taken to the lab. A lab requisition must <br />accompany the speci.men (This is completed in EHS for a SJWS.employee <br />or ordered-through the computer for an Occupational Health client). <br />The requisitian is ti.me-marked in the labaratory and initialled by <br />the perspn who delivers and the person who-receives the urine specimen. <br />13.
