2. If the average cost or premium is established to be <br />less than $470.00 per month over the above six month <br />period, the employee who paid with a paproll deduction <br />contribution shall be entitled to a refund determined by <br />the difference between $470.00 and the actual average cost <br />or premium times six months, but in no event greater than <br />that employee's contributions during the period specified <br />above. <br />All newly hired employees must have participated with a <br />payroll deduction during the entire six months to be <br />eligible for a refund. Any employee who changes from <br />single to family status or vice-versa during the above <br />period shall be entitled to a refund calculated only by <br />the number of months for which contribution was made. <br />3) If the average cost or premium should exceed $470.00 <br />per month for the six month period, the additional amount <br />shall be payed at a ratio of two-thirds by the City and <br />one-third by the employee. In this event, either the Citp <br />or the Association oay seek to reopen Article 19 by <br />notifying the other partp and SERB bp certified mail. <br />C. All changes affecting coverage or chosen providers shall be made <br />by a committee established as follows: one representative from each <br />of the City's bargaining units, if thep chose to be represented; one <br />representative from the Citp if there are one, two, or three <br />bargaining units represented and two representatives are behalf of <br />the City if there are more than three bargaining units on the <br />committee.
