2. If the average cost or premium is established to be less than $470.00 per month over the above six month period, the employee who paid with a paproll deduction contribution shall be entitled to a refund determined by the difference between $470.00 and the actual average cost or premium times six months, but in no event greater than that employee's contributions during the period specified above. All newly hired employees must have participated with a payroll deduction during the entire six months to be eligible for a refund. Any employee who changes from single to family status or vice-versa during the above period shall be entitled to a refund calculated only by the number of months for which contribution was made. 3) If the average cost or premium should exceed $470.00 per month for the six month period, the additional amount shall be payed at a ratio of two-thirds by the City and one-third by the employee. In this event, either the Citp or the Association oay seek to reopen Article 19 by notifying the other partp and SERB bp certified mail. C. All changes affecting coverage or chosen providers shall be made by a committee established as follows: one representative from each of the City's bargaining units, if thep chose to be represented; one representative from the Citp if there are one, two, or three bargaining units represented and two representatives are behalf of the City if there are more than three bargaining units on the committee. The committee representatives shall be chosen bp the respective parties and the committee, made up of the bargaining units which chose to do so, established no later than Januarp 31, 1992.
