SIIPPLEMENT TO HEALTH BENEFITS, ARTICLE%I% <br />AS BECOMISNDED BY FACT-FINDEB, DANA CASTLE <br />? <br />ABTICLE %I% <br />HSALTH BENEFIT <br />Section 19.1 The Citp shall provide either individual or family <br />medical insu=ance coverage, at the employees option, for each full- <br />time employee. The City and the Association, in accordance with the <br />plan specified below (Section C), shall have the option of seeking <br />alternate health insurance providers or plans and will attempt to <br />offer at Ieast two different plans. Basic hospitalization will be <br />consistent with and similar to policies/plans offered on December 1, <br />1991. <br />a. Period of Januarv 1- June 30, 1992 <br />I) Effective during January I-June 301 1992, the emploper <br />papment for all medical, hospital, prescription drug and <br />related fees shall be $425.00 per month. A paproll <br />deduction of $15.00 per month shall be made for sIl <br />emplopees in plans which have an expected cost or rate in <br />excess of $425.00 per month. <br />2) If the average cost or premium is established to be <br />Iess than $440.00 per month over the above six month <br />period, the emplopee who paid with a paproll contribution <br />shall be entitled to a refund determined bp the difference <br />between $440.00 and the actual average cost or premium <br />times six months, but in no event greater than that <br />emplopee's contributions during the period specified above. <br />All newly hired emplopees must have participated with apayroll deduction during the entire six months to be <br />eligible for a refund. Any emplopee who changes from <br />single to familp status or vice-versa during Lhe above <br />period shall be entitled to a refund calculated onlp bp the <br />number of months for which contribution was made. <br />3) If the average cost per month should exceed $440.00 per <br />month for the six month period, the additional amount shall <br />be paid at a ratio of two-thirds by the Citp and one-third <br />bq the emplopee. <br />b. Period of Julp 1- December 31, 1992 <br />1) Effective during Julp 1-December 31, 1992, the <br />enployer payment for all medical, hospital, prescription <br />drug and r.elated fees shall be $430.00 per month. A <br />paproll deduction of $40.00 per month shall be made for all <br />emplopees in plans which have an expected cost or ratfl in <br />exces3 of $430.00 per month.
