? 19.3 If the average actual monthly cost of coverage for the foliowing <br />periods is less than the respective Figure I for that period, the <br />Finployer shall refund to each contributing Finployee the difference <br />between the average actual cost and Fi^ure I for each month the <br />Friployee contriDUted. If the average actual cost falls between <br />the respective Fiorures I and II for that period, the Flnployer <br />shall pay 75% of the additional cost above Figure I and the <br />Eaaployee shall pay 25% of the additional cost. Refunds granted pursuant <br />to this section sha.ll not exceed the Fmployee's contribution <br />during that i.?eriod. If a refund is to be paid, it shall be paid <br />isithin 30 days of the determination of the actual cost for a <br />period. If an additional payroll deduction is required, it shall <br />be made within 30 days of the deterr,iination of the actual cost for <br />a period. <br />19.4 7he Friployer will make an earnest effort to establish a committee <br />consisting of one voti.ng representative fram each of the City <br />bargaining tmits, if they choose to be represented and a minimtnn <br />of one representative of the Fmployer. This bargaining unit's <br />role as a participant in the above committee shall be advisory <br />only. The Employer reserves the right to establish all plans, <br />enrollment periods and regulations. <br />All changes affecting coverage or chosen providers shall be shared <br />with the bargaining unit prior to making of such changes. <br />19.5 r.xpected cost or premitsn is defined as the cost of all medical, <br />hospital, prescription drug costs, and related fees. <br />19.6 Al1 Fmployees shall receive $15,000 in life insurance paid for by <br />the Fmployer. <br />?i -19-
