The Employer has the right to establish <br />the plan and to negotiate plan costs with providers for <br />hospitalization and prescription drug rates. <br />Effective during the period of July 20, 1992 through <br />December 31, 1992, the employer shall provide medical, <br />hospital, prescription drug and related fees without a <br />payroll deduction contribution by the employee. Effective <br />January 1, 1993 the maximum employer contribution for all <br />medical, hospital, prescription drug and related fees shall <br />be $430.00 per month for all employees enrolled in plans <br />which have an expected cost or premium of $430.00 per month, <br />a payroll deduction of $ 40.00 per month shall be made by all <br />employees enrolled in plans which have an expected cost or <br />premium of $430.00 per month or greater. . <br />If it is determined at the conclusion of the <br />hospitalization plan year that the average cost for medical, <br />hospital, prescription drug and all related fees was less <br />than $470.00 per month from July 1, 1992 through June 30, <br />1993,. the employee who paid with a payroll deduction <br />contribution shall be entitled to a refund determined by the <br />difference between $470.00 and the actual average cost or <br />premium, but in no event shall the refund be greater than <br />that employees contribution during the period January 1, 1993 <br />to June 30, 1993. Employees who may be entitled to a refund <br />shall receive such refund within (30) thirty days following <br />the determination on the actual average cost for the period <br />July 1, 1992 through June 30, 1993. <br />Al1 newly hired employees must have participated in the <br />plan along with a payroll deduction during the entire period <br />to be eligible for a refund. Any employee who changes from a <br />single to a family plan status or vice-versa during the above <br />period shall only be entitled to a refund calculated only by <br />the number of months for which contribution was made. <br />If the actual average cost or premiwa cost per month <br />exceed $470.00 per month, the additional amount shall be paid <br />at a ration of three-quarters (75%) by the City and one- <br />quarter (25%) by the employee. The employee share of the <br />cost which exceeds an average of $470.00 shall be withheld as <br />an additional payroll deduction within thirty (30) days <br />following the determination of the actual-average cost. <br />25 <br />...?? . _
