pursuant to this section shall not exceed the employee's <br />contribution during that period. If a refund is to be paid, it <br />shall be paid within 30 days of the determination of the actual <br />cost for a period. If an additional payroll deduction is <br />required it shall be made within 30 days of the determination of <br />the actual cost for a period. <br />Section 4: The employer will make an earnest effort to <br />establish a committee consisting of one (1) voting representative <br />from each of the City bargaining units, if they choose to be <br />represented and a minimum of one (1) representative of the <br />employer. This bargaining unit's role as a participant in the <br />above committee shall be advisory only. The employer reserves <br />the right to establish all plans, enrollment periods and <br />regulations. <br />All changes affecting coverage or_chosen providers shall be <br />shared with the Health Care Committee prior to making of such <br />changes. <br />Section 5: Expected cost or premium is defined as the cost <br />of all medical, hospital, prescription drug costs, and related <br />fees, but does not include any cost of the AFSCME Care Plan. <br />Section 6: After consultation with the hospitalization <br />committee established in Section 4`'above,' I 'the employer reserves <br />the right to establish office co-payments, prescription drug co- <br />payments, and deductibles if it is determined that the actual <br />cost or premium cost per month as"defined in this article exceed: <br />$525 per month for the period July 1, 1993 through June 30, <br />1994 for employees subscribing to the family plan <br />$170 per month for the period July l, 1993 through June 30, <br />1994 for employees subscribing to the single plan <br />34
