Such pa.yment shall be made only for those sick hours earned by the employee <br />while employed by the employer. Sick pay shall be based on the employee's <br />rate of pay at the time of conversion. Payment for sick leave, on this <br />basis sha.ll be considered to eliminate all sick leave credit accrued by the <br />employee. Such payment shall be made only once to an employee. <br />Section 18.14. Sick leave can be used for the following reasons: <br />(a) Sickness of the employee; <br />(b) Injury to the employee; <br />(c) Exposure to contagious disease where quarantined by the Board of <br />Health; <br />(d) Fmergency/non-routine medical, dental surgery or optical surgery <br />of the employee; and <br />(e) Sickness or emergency medical treatment of the employee's <br />"immediate family" shall be interpreted as meaning parents, <br />parents of spouse, spouse, and children of the employee, <br />regardless of place of residence, as well as other relatives <br />living in the employee's home. The maximiun sick leave which may <br />be granted under,this paragraph for family illness may be no more <br />than eighty (80) hours per year for enployees who ha.ve been with <br />the City more than twelve (12) months. Department heads may <br />require a satisfactory medical certificate before paying any <br />employee under this paragraph. Falsification of either a <br />written, signed statement or a medical certificate shall be <br />grounds for disciplinary action including dismissal. <br />(f) Sick leave may be used to attend the funeral of a half-brother, <br />half-sister, brother-in-law or sister-in-law, for a max.i_mm of <br />three (3) days. <br />Section 18.15. Any abuse of sick leave or the pa.tterned use of sick <br />leave shall be just and sufficient cause for discipline, including <br />discharge, as may be determined by the Fmployer. (Example: Mondays, <br />Fridays, day after payday, etc.) <br />Section 18.16. Any enployee scheduled to work on a holiday who reports <br />sick shall be charged sick leave for that holiday, provided the proper NOMBL <br />medical form has been completed and shall forfeit his right to receive the <br />holiday pay. If the employee has no sick leave or does not have the proper <br />NOMBL medical form completed, he shall receive no pa.y for that day. <br />ARTICLE XIX <br />LEAVES OF ABSENCE & FUNERAL LEAVES <br />Section 19.01. All employees shall be entitled to take a personal <br />leave of absence with the approval of the Service Director and tmder the <br />terms and conditions as he sets forth, which approval shall not be <br />unreasona,bly withheld. Any leave of absence for personal reasons, shall <br />not, however, exceed a ninety (90) day period. Said employee in order to <br />maintain any and a11 benefits which are paid by the City shall be required <br />to pay the Finance Director, the cost of said benefits in advance on a <br />monthly basis. The maximum permissible leave of absence for any reason <br />sha.ll be ninety (90) days. <br />-14-
