Section 23.06. Effective January 1, 1992, any full-time employee who is eligible for vacation allowance of three (3) weeks or more may request payment of one (1) week in lieu of tirne off. Such request must be made in writing, at the time of his vacation pick, provided that such request is ma.de at least fourteen (14) days prior to the applicable pay period for payment. This one (1) week payment will be made no later than the first pay period in Deceznber of each year. For purposes of this section, a week is defined at 40 hours. ARTICLE XXIV HRAT,TH INSURANCE Section 24.01. The Eriployer shall provide either individual or family medical insurance coverage, at the employee`s option for each full-time enployee. Effective August l, 1991 through December 31, 1991; the Fmployer shall pay a maximun of $370.67 toward the monthly health premiiun including riders (dental, vision, drugs). The Employer shall have the option of seeking alternate health insurance providers and will attempt to continue to offer at least two different plans. Sectian 24.02. Effective January, 1992, the Enployer shall provide either individual or family medical insurance coverage at the employee's option for each full-time employee. The Employer sha.ll ha.ve the option of seeking alternate health insurance providers or plans and will attempt to offer at least two different plans. Basic hospitalization will be consistent with and similar to those plans offered an December 1, 1991. Basic hospitalization will be that offered to other employees of the City of North Olmsted. The Enployer has the right to negotiate co-payment and prescription drug rates with providers to lower the cost to the employer as well as the employee contribution. Section 24.03. PERIOD OF JANUARY 1, 1992 TO JUNE 30, 1992 Effective during January 1, through June 30, 1992, the maxi.minn employer contribution for a11 medical, hospital, prescription drug and related fees shall be $410.00 per month. A payroll deduction of $30.00 per month shall be made by a11 enployees in plans which ha.ve an expected cost or premiun in excess of $440.00 per month. ' If the average cost or premium is established to be less tYian $440.00 per month over the above (6) six-month period, the employee who paid with a payroll deduction contribution shall be entitled to a refund determined by the difference between $440.00 and the actual average cost or premiun times (6) six months, but in no event greater than that employee's contribution durirg the period specified above. Fmployees who may be entitled to a refund shall receive such refund within thirty (30) days following the determination of the actual average cost for the period, January l, 1992 through June 30, 1992. -19-
