Section 23.06. Effective January 1, 1992, any full-time employee <br />who is eligible for vacation allowance of three (3) weeks or more may <br />request payment of one (1) week in lieu of tirne off. Such request must <br />be made in writing, at the time of his vacation pick, provided that such <br />request is ma.de at least fourteen (14) days prior to the applicable pay <br />period for payment. This one (1) week payment will be made no later <br />than the first pay period in Deceznber of each year. For purposes of <br />this section, a week is defined at 40 hours. <br />ARTICLE XXIV <br />HRAT,TH INSURANCE <br />Section 24.01. The Eriployer shall provide either individual or <br />family medical insurance coverage, at the employee`s option for each <br />full-time enployee. Effective August l, 1991 through December 31, 1991; <br />the Fmployer shall pay a maximun of $370.67 toward the monthly health <br />premiiun including riders (dental, vision, drugs). The Employer shall <br />have the option of seeking alternate health insurance providers and will <br />attempt to continue to offer at least two different plans. <br />Sectian 24.02. Effective January, 1992, the Enployer shall provide <br />either individual or family medical insurance coverage at the employee's <br />option for each full-time employee. The Employer sha.ll ha.ve the option <br />of seeking alternate health insurance providers or plans and will attempt <br />to offer at least two different plans. Basic hospitalization will be <br />consistent with and similar to those plans offered an December 1, 1991. <br />Basic hospitalization will be that offered to other employees of the City <br />of North Olmsted. The Enployer has the right to negotiate co-payment and <br />prescription drug rates with providers to lower the cost to the employer <br />as well as the employee contribution. <br />Section 24.03. PERIOD OF JANUARY 1, 1992 TO JUNE 30, 1992 <br />Effective during January 1, through June 30, 1992, the maxi.minn employer <br />contribution for a11 medical, hospital, prescription drug and related <br />fees shall be $410.00 per month. A payroll deduction of $30.00 per month <br />shall be made by a11 enployees in plans which ha.ve an expected cost or <br />premiun in excess of $440.00 per month. ' <br />If the average cost or premium is established to be less tYian $440.00 per <br />month over the above (6) six-month period, the employee who paid with a <br />payroll deduction contribution shall be entitled to a refund determined <br />by the difference between $440.00 and the actual average cost or premiun <br />times (6) six months, but in no event greater than that employee's <br />contribution durirg the period specified above. Fmployees who may be <br />entitled to a refund shall receive such refund within thirty (30) days <br />following the determination of the actual average cost for the period, <br />January l, 1992 through June 30, 1992. <br />-19-
